Isolated systolic hypertension is typical for the elderly, but also occurs in younger adults. Increased pulse wave amplification between the elastic aorta and arteries to the arm can result in a higher peripheral (brachial) blood pressure, while central (aortic) systolic blood pressure is normal. A 21-year-old athletic man was referred because of an arterial blood pressure of 160/85 mmHg.

Objectives: Glucocorticoids (GC) are important in the treatment of autoinflammatory disorders. Oral prednisolone ≤5 mg/day can be effective, but such doses are at or even below physiological daily endogenous GC production. We hypothesised that their immunosuppressive effect might be explained by high hepatic bioavailability of oral GC, exposing the liver to supraphysiological GC via the portal circulation.

Article Synopsis
  • Most labs in The Netherlands provide an estimated glomerular filtration rate (eGFR) when plasma creatinine levels are checked, using a formula based on factors like gender, age, and sometimes race.
  • This eGFR reporting has helped identify chronic kidney disease better, but the formulas have their limitations.
  • A new formula has been suggested that eliminates the need for a race correction factor, and the article discusses the pros and cons of using plasma creatinine formulas, along with answers to ten common questions.

Article Synopsis
  • The study evaluated the effectiveness of a stepwise approach for treating cardiovascular risk factors based on European guidelines in patients with established atherosclerotic cardiovascular disease (ASCVD).
  • Researchers analyzed data from two large patient cohorts, identifying treatment goals focused on lowering LDL cholesterol and blood pressure, as well as promoting smoking cessation and various medication combinations.
  • Results indicated that using intensified Step 2 treatment options significantly reduces residual cardiovascular risk, improving patient outcomes and potentially decreasing events per 1000 patients treated in different European risk regions.

Clinical practice guidelines are fundamental to support knowledge and decision making of healthcare professionals in many disciplines. They can contribute to reducing undesirable practice variation, educating patients, and monitoring care. However, the burden increases if the number and size of guidelines continues to increase and as more side effects occur due to injudicious use, both in the professional and policy setting.

Article Synopsis
  • - The study aimed to evaluate the impact of 4 years of anti-inflammatory therapy on subclinical vascular disease markers in rheumatoid arthritis (RA) patients, comparing them to controls with osteoarthritis (OA).
  • - Results showed that RA patients had higher carotid intima media thickness (IMT) and augmentation index (AIx@75) at baseline compared to OA, but anti-inflammatory treatment led to significant initial improvements in AIx@75 and pulse wave velocity (PWV) within the first 6 months, although PWV returned to baseline levels by 48 months.
  • - The findings indicate that anti-inflammatory therapy resulted in modest improvements in certain vascular disease markers, which were related to decreased disease activity, yet the long-term

Estimation of 10-year cardiovascular risk has a central role in Cardiovascular Prevention Guidelines in order to identify people at high risk that would benefit from lifestyle modification and risk factor treatment. In the recently issued European Guidelines age-dependent 10-year risk thresholds have been introduced, as well as lifetime risk estimation and estimation of treatment benefit, which all can be used in clinical practice to further individualize treatment of cardiovascular risk factors. Also cardiovascular risk prediction in people with diabetes and patients with already established cardiovascular disease are now available to estimate residual cardiovascular risk after initial lifestyle and risk factor treatment to guide individual intensification of risk factor treatment to further reduce the risk of a first or subsequent cardiovascular event.

Study Objective: To compare the prognostic accuracy of clinical judgment for frailty in older patients at the emergency department with a validated screening instrument and patient-perceived frailty.

Methods: A prospective cohort study in patients 70 years of age and older in 2 Dutch EDs with a follow-up of 3 months. A dichotomous question was asked to the physician and patient: "Do you consider the patient / yourself to be frail?" The Identification of Seniors At Risk-Hospitalized Patients (ISAR-HP) was used as a validated screening instrument.

Aims: To develop a set of quality indicators (QIs) for the evaluation of the care and outcomes for atherosclerotic cardiovascular disease (ASCVD) prevention.

Methods And Results: The Quality Indicator Committee of the European Society of Cardiology (ESC) formed the Working Group for Cardiovascular Disease Prevention Quality Indicators in collaboration with Task Force members of the 2021 ESC Guidelines on Cardiovascular Disease Prevention in Clinical Practice and the European Association of Preventive Cardiology (EAPC). We followed the ESC methodology for QI development, which involved (i) the identification of the key domains of care for ASCVD prevention by constructing a conceptual framework of care, (ii) the development of candidate QIs by conducting a systematic review of the literature, (iii) the selection of the final set of QIs using a modified Delphi method, and (iv) the evaluation of the feasibility of the developed QIs.
