The prevalence of renal colic is increasing in industrialized countries, representing a frequent reason for consultation in emergencies. Most patients have simple renal colic that will require analgesia and ambulatory monitoring. Doctors working in emergency centers play a key role in the diagnosis, care and guidance of these patients.

Sonography showed a hydrocele and a homogeneous, well-encapsulated left extratesticular mass with similar echogenicity as the normal testis, suggestive of a splenogonadal fusion. To substantiate the diagnosis, the patient underwent Tc-99m heat-denatured red blood cell scintigraphy showing normal physiological hyperactivity in the spleen but activity similar to the blood pool projecting on the upper part of the left testis.

To choose the best attitude in front of the obstruction of the higher urinary tract secondary at a pelvic or retroperitoneal tumor is delicate and very often requires a multidisciplinary approach. The mains indications on the invasive minimal urinary derivations are the prolongation of life expectancy by the safeguarding of the renal function and the improvement of the quality of life during the presence of pains. The opinion of the patient is central to make decision.
