A 67-year-old man visited our hospital for epigastric pain. Esophagogastroduodenoscopy(EGD)revealed type 2 gastric cancer from the cardia to the gastric angle, and histopathological examination revealed papillary adenocarcinoma(pap), HER2-positive. Contrast-enhanced CT showed wall thickening mainly in the posterior wall of the gastric body, enlarged lymph nodes that were lumped together with the main lesion, and 8 low-absorption areas with ring shaped contrast effects in both lobes of the liver.

Background: A leiomyosarcoma (LMS) is a rare tumor that mainly originates from the urinary tract and digestive system; however, non-visceral organ-derived patterns are rare. Herein, we report that a vessel-derived pleomorphic LMS (PLMS) originating from the mesenteric vein has a poor prognosis even after curative-intent surgery.

Case Presentation: The patient was a 41-year-old woman with no relevant medical history.

This case report describes a patient with nonocclusive mesenteric ischemia that developed due to diabetic ketoacidosis. We believe that early diagnosis and intervention may improve the prognosis of nonocclusive mesenteric ischemia that has low vascular risk, with the major risk factor being dehydration due to diabetic ketoacidosis.

Background: Anastomotic leakage (AL) occurs with some frequency in all types of colorectal cancer surgery and is associated with increased morbidity, mortality and recurrence rates. Complications might be prevented by monitoring intra-operative bowel perfusion at the anastomotic site. A pilot study concerning the objective and quantitative measurement of tissue perfusion by monitoring regional tissue saturation of oxygen (rSO) was conducted, using the In Vivo Optical Spectroscopy (INVOS™) system (Medtronic, Minneapolis, MN, USA).

Article Synopsis
  • The study investigates how residual pneumoperitoneum (remaining air in the abdominal cavity) after laparoscopic colorectal surgery affects patient recovery.
  • It analyzes data from 201 Japanese patients, comparing those with and without anastomotic failure, and further categorizing non-anastomotic failure patients by the presence of residual pneumoperitoneum.
  • Key findings indicate that patients with residual pneumoperitoneum tend to have lower body mass index (BMI) and subcutaneous fat area (SFA), and highlight inflammatory markers as potential indicators for complications.

Background: Hepatectomy for resectable colorectal liver metastasis (CRLM) is recommended. However, the efficacy of upfront hepatectomy without neoadjuvant chemotherapy (NAC) is unclear due to the uncertainty of perioperative systemic chemotherapy. Moreover, it is crucial to predict the prognosis when considering perioperative chemotherapy.

Purpose: Acute peritonitis has remained a fatal disease despite of recent advances in care and treatment, including antibiotic and anticoagulant treatments. The cause of death is mostly sepsis-induced multiple organ failure. Oxidative stress can play an important role in this situation, but antioxidant therapy to capture any excessive reactive oxygen species has not yet been fully established.

Background: We focused on the availability of an omnidirectional camera and head-mount display (HMD). If the laparoscope is an omnidirectional camera, captured images are sent to the HMD worn by the operator in real time. The operator can thus view the image as they like without moving the camera and obtain a 360° view intuitively.

Article Synopsis
  • The study investigates the prevalence and predictors of fecal incontinence (FI) following very low anterior resection (VLAR) for rectal cancer, as FI significantly impacts patients' quality of life (QOL).
  • Analysis of anorectal manometry data from 40 patients revealed that low preoperative incremental maximum squeeze pressure (iMSP) is a significant risk factor for developing major FI post-surgery.
  • The findings emphasize the necessity of preoperative evaluations to assess anal function, helping to guide surgical decisions and inform early treatments for those at risk of postoperative FI.

Purpose: The number of patients on antiplatelet therapy (APT) who need surgery is increasing; however, it is unclear whether APT should be continued for abdominal surgery, particularly laparoscopic colorectal surgery. We investigated the safety of continuing APT for patients undergoing laparoscopic colorectal surgery.

Methods: We collected retrospective data from 529 patients who underwent laparoscopic colorectal surgery at Hiroshima University between January, 2013 and December, 2018.

Article Synopsis
  • - A rare case of ascending colon cancer complicated by tumor thrombosis in the superior mesenteric vein (SMV) was reported with successful treatment involving complete resection.
  • - A 48-year-old woman was treated for anemia and diagnosed with the cancer and thrombosis, leading to surgery that included hemicolectomy and tumor thrombectomy, followed by chemotherapy.
  • - The study suggests that removing the primary cancer and addressing tumor thrombosis, along with chemotherapy, can improve patient outcomes and reduce the likelihood of recurrence.

A novel disulfide, which carried two pepstatin fragments at both ends, was prepared by the coupling of 11,11'-dithiobisundecanoic acid (DTUA) with a fragment (Val-Val-Sta) carrying a n-hexyl end (Pepsta(h)). The compound obtained (DTUA-Pepsta(h)) formed a self-assembled monolayer (SAM) on a gold electrode and vacuum-evaporated gold thin film as proven by cyclic voltammetry and reflection absorption infrared spectroscopy, respectively. When the SAM-modified gold electrode was incubated with a solution of aspartyl protease, pepsin, a decrease in both anodic and cathodic peak currents and an increase in potential difference were observed in the cyclic voltamogram of hydroquinone as a probe, whereas a coexistence of free pepstatin fragment inhibited these phenomena, indicating the specific binding of pepsin to the fragment at the exterior of the SAM.
