Article Synopsis
  • * A total of 7062 patients participated, and the results showed no significant difference in primary cardiovascular outcomes between the colchicine group (9.1%) and the placebo group (9.3%) over a 3-year follow-up period.
  • * Colchicine did lower C-reactive protein levels, indicating some anti-inflammatory effect, but it also caused more diarrhea compared to placebo, though serious infections were similar in both groups.

Monitoring the conduct of phase III randomized controlled trials is driven by ethical reasons to protect the study integrity and the safety of trial participants. We propose a group sequential, pragmatic approach for monitoring the accumulating efficacy information in randomized controlled trials. The "Population Health Research Institute boundary" is simple to implement and sensible, as it considers the reduction in uncertainty with increasing information as the study progresses.

Objectives: Long-term exposure to air pollution has been associated with higher risk of cardiovascular mortality. Less is known about the association of air pollution with initial development of cardiovascular disease. Herein, the association between low-level exposure to air pollutants and subclinical carotid atherosclerosis in adults without known clinical cardiovascular disease was investigated.

Background: The built environment can influence human health, but the available evidence is modest and almost entirely from urban communities in high-income countries. Here we aimed to analyse built environment characteristics and their associations with obesity in urban and rural communities in 21 countries at different development levels participating in the Prospective Urban and Rural Epidemiology (PURE) Study.

Methods: Photographs were acquired with a standardised approach.

Article Synopsis
  • The study aims to investigate the prevalence of activity limitations and the use of assistive devices across 25 countries with varying economic statuses, focusing on how these limitations correlate with negative health outcomes.
  • Researchers analyzed data from the Prospective Urban Rural Epidemiological (PURE) study, which involved over 175,000 participants aged 35-70, looking specifically at self-reported difficulties in daily functions, as well as demographic factors.
  • The findings highlight a gap in understanding the relationship between activity limitations and mortality or clinical events, revealing the need for more comprehensive studies on how such limitations impact health across different populations.

Background: Cardiovascular disease (CVD) continues to impart a large burden on the global population, especially in lower income countries where affordability limits the use of cardiovascular medicines. A fixed dose combination strategy of at least 2 blood pressure lowering medications and a statin with aspirin in a single pill has been shown to reduce the risk of incident CVD by 38% in primary prevention in a recent meta-analysis. We report the in-trial (median follow-up: 5 years) cost-effectiveness of a fixed dose combination (FDC) pill in different income groups based on data from that meta-analysis.

Article Synopsis
  • Concerns have emerged regarding the effectiveness of renin-angiotensin system (RAS) blockers in treating heart failure in Black patients compared to non-Black patients with reduced ejection fraction.
  • A study analyzed randomized trials involving both Black and non-Black adults with heart failure to see how RAS blockers affect cardiovascular outcomes between these two groups.
  • Results indicated that Black patients had higher rates of death and hospitalization for heart failure than non-Black patients, with less impactful effects of RAS blockers observed in Black patients, particularly concerning hospitalizations for heart failure.

Article Synopsis
  • Cognitive decline is a major concern with aging, linked to higher risks of dementia and stroke, though the exact causes are unclear.
  • A study of 1160 proteins in plasma found five that are associated with cognitive function, specifically in a test called DSST, with two proteins (NCAN and CDCP1) showing consistent results in a follow-up group.
  • The research indicates that some proteins may influence brain health and stroke risk, suggesting avenues for future research and potential drug treatments to help mitigate cognitive decline and related risks.
