Article Synopsis
  • Differentiating left ventricular hypertrophy (LVH) from conditions like hypertrophic cardiomyopathy (HCM) and Fabry disease is complex and often requires input from various specialists, which leads to inconsistent diagnoses.
  • *A new AI-based tool called the MRI short-axis view left ventricular hypertrophy classifier (MSLVHC) has been developed to accurately distinguish HCM from Fabry disease using MRI cine images and has shown high accuracy and strong performance metrics.
  • *The model not only demonstrated reliability in tests at two hospitals but also holds potential to enhance the diagnostic process for specialists dealing with LVH-related conditions.

Article Synopsis
  • Acute coronary syndrome (ACS) in young adults under 40 may be linked to previously undiagnosed Kawasaki disease (KD) from childhood, with studies indicating a potential prevalence of 7.4% in patients with coronary aneurysms.
  • A multicenter study in Taiwan reviewed records from 323 young adults who underwent coronary angiography, revealing that most patients with aneurysms were male and had higher LDL cholesterol levels.
  • The findings suggest that higher incidence rates of KD in certain populations could correlate with increased cases of ACS in young adults, emphasizing the need for further research into the long-term cardiovascular effects of KD.

Background/aim: Identifying pathogens with culture-negative pyogenic spondylitis is difficult. Shotgun metagenomic sequencing is an unbiased and culture-free approach in the diagnosis of infectious diseases. There are, however, a variety of contaminating factors that can confound the precision of metagenomic sequencing.

Infective endocarditis is a potentially fatal condition, and identifying the pathogen is crucial to optimizing antibiotic treatment. While a blood culture takes time and may yield negative results, it remains the gold standard for diagnosis, blood culture-negative endocarditis, which accounts for up to 20% of infective endocarditis cases, poses a clinical challenge with increasing mortality. To better understand the etiology of blood culture-negative infective endocarditis, we reviewed non-culture-based strategies and compared the results.

Article Synopsis
  • Psychiatric symptoms, like acute delirium, are commonly associated with SARS-CoV-2 infection and can be mistaken for affective disorders, complicating diagnosis.
  • In two reported cases, acute delirium was the first sign of acute ischemic stroke (AIS), but the diagnosis was delayed due to the misinterpretation of symptoms.
  • Brain MRI ultimately confirmed AIS despite challenges in diagnosis, highlighting the need to consider potential strokes in COVID-19 patients with elevated serum D-dimer and psychiatric symptoms.

Thromboembolism is a critical event in patients with coronavirus disease (COVID)-19 infection and highly associated with neutrophil extracellular traps. D-dimer has been found to be an essential thromboembolism-associated biomarker; however, the association between absolute neutrophil count (ANC) and level of D-dimer in patients with COVID-19 infection remains unclear. In this study, we enrolled consecutive patients with COVID-19 admitted to Taichung Veterans General Hospital (TCVGH), a referral center in central Taiwan with 20 airborne infection isolation rooms.

N-terminal pro b-type natriuretic peptide (NT-proBNP) was considered a prognostic factor for mortality in hemodialysis patients in previous studies. However, NT-proBNP has not been fully explored in terms of predicting other clinical outcomes in hemodialysis patients. This study aimed to investigate if NT-proBNP could predict emergency department (ED) visits, hospitalization, admission to intensive-care unit (ICU), and cardiovascular incidents in hemodialysis patients.
