Introduction: The Hugo robotic-assisted surgery (RAS) system is a novel surgical platform that expands the potential for minimally invasive procedures in gynecologic oncology. Early studies have shown promise, but the optimal configuration of ports and robotic arms requires further clarification. This study aimed to introduce a refined strategy for optimizing port placement and robotic arm arrangement with the Hugo RAS system and compare its performance to previously employed configurations.

Background: This study evaluated the effectiveness of preoperative inflammatory response markers in distinguishing clear cell carcinoma (CCC) and endometrioid carcinoma (EC) from ovarian endometrioma.

Methods: Patients with stage I ovarian cancer with histology CCC/EC or endometrioma who underwent surgery at our institution between 2010 and 2021 were included. Preoperative inflammatory response markers evaluated were white blood cell count, C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), fibrinogen, D-dimer, neutrophil/lymphocyte ratio (NLR), platelet count/lymphocyte ratio (PLR), and systemic immune-inflammation index (SII).

Background: Pose estimation algorithms applied to two-dimensional videos evaluate gait disturbances; however, a few studies have used this method to evaluate ataxic gait.

Objective: The aim was to assess whether a pose estimation algorithm can predict the severity of cerebellar ataxia by applying it to gait videos.

Methods: We video-recorded 66 patients with degenerative cerebellar diseases performing the timed up-and-go test.

Aim: In the PEMA-FL study in patients with metabolic dysfunction-associated steatotic liver disease (MASLD), pemafibrate was shown to significantly decrease low-density lipoprotein cholesterol (LDL-C) levels. We aimed to investigate the mechanisms of pemafibrate-induced LDL-C reduction in patients with MASLD by conducting an additional sub-analysis of the PEMA-FL study.

Methods: The PEMA-FL study randomized 118 patients with MASLD to receive pemafibrate or placebo for 72 weeks.

Article Synopsis
  • This study evaluated the practical application of cardiovascular physical therapy for older heart failure patients in Japan, utilizing data from a national registry of hospitalized patients aged 65 and older.
  • The assessment showed a high completion rate for evaluations, with over 80% successfully completing physical therapy; however, gait speed and short-performance tests had lower completion rates for those with lower Barthel Index scores.
  • The findings suggest that while routine assessments of medical conditions are feasible in hospitals, interventions focused on gait and muscle strength training are more prevalent than aerobic or resistance exercise among these patients.

Recently, a Letter to the Editor critiquing the recommendations of the Japanese Clinical Practice Guidelines for Rehabilitation in Critically Ill Patients, 2023, was published. The comment centered on the recommendation, "Weak recommendation against the use of endoscopy-based management (GRADE 2D: certainty of evidence = 'very low')" for the clinical question, "Should critically ill patients be managed based on video endoscopic assessment of swallowing?" In response, we outline the rationale behind our recommendations and their clinical implications.

  • The study investigates the use of pose estimation algorithms to distinguish between Parkinson's disease (PD) and spinocerebellar degeneration (SCD) through video analysis of patient gait.
  • Researchers recorded 82 PD patients and 61 SCD patients during a mobility test and utilized a deep neural network model trained on gait data to make predictions on the neurological disorder.
  • Results indicated high accuracy (0.86), sensitivity (0.94), and specificity (0.75) for differentiating PD from SCD, suggesting this method could be valuable in clinical settings and telemedicine for improved patient assessment.

Introduction: Hybrid total laparoscopic hysterectomy combines conventional laparoscopic surgery and robot-assisted devices: the camera and assistant forceps are operated by a robotic device, whereas the surgeon performs laparoscopic procedures, enabling surgery with a completely fixed field of view and significantly reducing errors in forceps grasping and needle misalignment. Here, we examined whether using two arms of the Hugo™ robot-assisted surgery system, one for the camera and one for the assistant, would improve surgical accuracy compared with conventional total laparoscopic hysterectomy.

Materials And Surgical Technique: The surgical system reduced surgeon errors in grasping the forceps during training and stabilized forceps operation.

  • Dysphagia, or difficulty swallowing, is common in critically ill patients, and various rehabilitation methods are used globally to treat it.
  • A review of randomized controlled trials found that dysphagia rehabilitation reduced the risk of pneumonia but did not significantly affect mortality or health-related quality of life.
  • The analysis included 19 trials with over 1,000 participants, indicating moderate certainty that rehabilitation helps with pneumonia but very low certainty concerning its effects on mortality and quality of life.

Background: Lung resection is the standard of care for patients with clinical stage I/II non-small cell lung cancer. This surgery reduces both the duration and quality of patients' daily ambulatory activities 1 month after surgery. However, little is known about physical activity after lung resection in patients with lung cancer.

Few studies have compared the efficacy of robot-assisted, laparoscopic, and open surgeries for endometrial cancer. When considering the position of robotic surgery in Japan, it was necessary to determine whether it was effective or not. We aimed to compare the efficacy and safety of these three types of surgeries for early-stage endometrial cancer.

Background And Objective: Interstitial lung disease (ILD) is characterized by dyspnoea on exertion and exercise-induced hypoxaemia. High-flow nasal cannula (HFNC) therapy reduces the respiratory workload through higher gas flow and oxygen supplementation, which may affect exercise tolerance. This study aimed to examine the effects of oxygen and gas flow rates through HFNC therapy on exercise tolerance in ILD patients.

Psychological aspects of rehabilitation nutrition affect physical, cognitive, and social rehabilitation nutrition. When depression is recognized, not only pharmacotherapy and psychotherapy, but also non-pharmacological therapies such as exercise, nutrition, psychosocial, and other interventions can be expected to improve depression. Therefore, accurate diagnosis and intervention without overlooking depression is important.

  • A study analyzed the incidence of hospitalization-associated disability (HAD) in older heart failure patients in Japan, finding that 37.1% of the 9,403 participants experienced HAD during their hospital stay.
  • Patients with HAD were generally older and had higher rates of comorbidities such as hypertension and chronic kidney disease, along with lower functional scores prior to admission.
  • The research suggests that factors like age and prehospital health status are linked to the development of HAD, highlighting the need for increased rehabilitation efforts for older heart failure patients in Japan.

Providing standardized, high-quality rehabilitation for critically ill patients is a crucial issue. In 2017, the Japanese Society of Intensive Care Medicine (JSICM) promulgated the "Evidence-Based Expert Consensus for Early Rehabilitation in the Intensive Care Unit" to advocate for the early initiation of rehabilitations in Japanese intensive care settings. Building upon this seminal work, JSICM has recently conducted a rigorous systematic review utilizing the Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) methodology.
