Introduction: Osimertinib is the first-line treatment for patients with non-small cell lung cancer (NSCLC) who have EGFR mutations and favorable performance status (PS). Despite increasing clinical data on osimertinib, evidence in patients with an impaired PS remains limited. Therefore, a multicenter phase II trial (OPEN/TORG2040) was conducted to evaluate the efficacy and safety of first-line osimertinib for patients with EGFR mutation-positive NSCLC and poor PS.

Article Synopsis
  • Sleep apnea (SA) and both subjective (SSD) and objective sleep durations (OSD) are linked to atrial fibrillation (AF), but their combined effects and the discrepancies between SSD and OSD haven't been thoroughly examined.
  • A study using polysomnography and actigraphy found that a significant portion of participants had moderate-to-severe SA and that those with longer OSD had fewer hours of SSD, potentially indicating poor sleep quality.
  • Findings revealed that those with higher SA severity and longer OSD were more likely to experience AF recurrence, suggesting that long exposure to oxygen fluctuations during sleep contributes to this risk.

Background: A unilateral absence of the pulmonary artery (UAPA) is a rare congenital anomaly, which in most patients is associated with congenital cardiovascular abnormalities. Some patients with UAPAs do not have associated cardiovascular abnormalities, called isolated UAPA. Patients with isolated UAPA generally have a mild clinical course and may not be diagnosed until adulthood, but repeated complications such as infections may make surgery difficult.

The striatum consists of two anatomically and neurochemically distinct compartments, striosomes and the matrix, which receive dopaminergic inputs from the midbrain and exhibit distinct dopamine release dynamics in acute brain slices. Striosomes comprise approximately 15 % of the striatum by volume and are distributed mosaically. Therefore, it is difficult to selectively record dopamine dynamics in striosomes using traditional neurochemical measurements in behaving animals, and it is unclear whether distinct dynamics play a role in associative learning.

Article Synopsis
  • * It analyzed 374 patients and found that certain scores, specifically the advanced lung cancer inflammation index and nonalcoholic fatty liver disease fibrosis score, significantly correlated with a higher incidence of complications, particularly in a high-risk group.
  • * The combined use of these scores not only improved predictive accuracy for complications but also suggested a potential influence on selecting appropriate surgical procedures, with higher complication rates seen in the anatomical resection vs. partial resection in high-risk patients.

Owing to the risk of atypical femoral fractures, total hip arthroplasty presents unique challenges for patients with ipsilateral osteoarthritis and localized periosteal thickening in the femoral subtrochanteric region. Stem length selection is critical for minimizing stress concentration in the thickened cortex to avoid such fractures. Herein, we report the case of a 78-year-old woman with ipsilateral hip osteoarthritis and localized subtrochanteric periosteal thickening.

Article Synopsis
  • The β-hydroxyacid dehydrogenase family shows varied preferences for cofactors NAD and NADP, with some enzymes able to use both; glyoxylate reductase from Acetobacter aceti JCM 20276 (AacGR) can utilize both NADPH and NADH for its reaction.
  • X-ray crystallography reveals that AacGR possesses a unique TPS motif that interacts differently with NADPH and NADH, aiding in its dual cofactor specificity.
  • Mutagenesis studies and kinetic analyses indicate that specific interactions with Ser41 in the TPS motif enhance NADPH binding, while the flexible structure allows NADH to bind without direct interaction, contributing to the enzyme's catalytic turnover.

Article Synopsis
  • A 20-year-old woman was hospitalized after consuming high doses of caffeine (18g) and mexiletine (3500mg), leading to severe hypotension, tachycardia, hypokalemia, and lactic acidosis.
  • She underwent multiple medical interventions, including mechanical ventilation, noradrenaline infusion, and hemodialysis (HD), as her condition worsened to pulseless ventricular tachycardia requiring VA-ECMO support.
  • The case highlighted that mexiletine may hinder caffeine clearance, complicating treatment; however, after HD, her drug levels decreased significantly, allowing successful recovery and weaning from life support within a few days.

Gene expression in response to environmental stimuli is dependent on nuclear localization of key signaling components, which can be tightly regulated by phosphorylation. This is exemplified by the phosphate-sensing transcription factor Pho4, which requires phosphorylation for nuclear export by the yeast exportin Msn5. Unlike the traditional hydrophobic nuclear export signal (NES) utilized by the Exportin-1/XPO1 system, cryogenic-electron microscopy structures reveal that Pho4 presents a novel, phosphorylated 35-residue NES that interacts with the concave surface of Msn5 through two Pho4 phospho-serines that align with two Msn5 basic patches, unveiling a previously unknown mechanism of phosphate-specific recognition.

  • LGR5 is a marker for cancer stem cells and its expression in extrahepatic cholangiocarcinoma (ECC) was studied using a sensitive RNA detection method in 53 cases.
  • The study found that higher LGR5 expression was linked to well- to moderately differentiated cancers, which showed better overall and disease-free survival rates compared to poorly differentiated cancers.
  • The results indicate that LGR5 could serve as a valuable prognostic marker for patients with ECC, potentially guiding treatment decisions.

Article Synopsis
  • The study compares anatomical liver resection (AR) and non-anatomical liver resection (NR) for treating localized hepatocellular carcinoma (HCC), finding that AR significantly reduces the rates of local and early recurrence.
  • In a matched analysis of 280 patients, AR led to lower recurrence rates and better overall survival compared to NR (13.8% vs. 28.7% for local recurrence).
  • The findings suggest that AR is a more effective surgical option for solitary HCC ≤5 cm, as it minimizes the chance of recurrence and improves long-term survival.
