Activation of the renin-angiotensin-aldosterone system (RAAS) plays an important pathophysiological role in hypertension. Increased mRNA levels of the , angiotensin type 1 receptor gene, , and the aldosterone synthase gene, have been reported in the heart, blood vessels, and kidneys in salt-sensitive hypertension. However, the mechanism of gene regulation in each component of the RAAS in cardiovascular and renal tissues is unclear.

Aldosterone secretion in primary aldosteronism (PA) is often regulated by adrenocorticotropic hormone (ACTH) in addition to its autonomous secretion. However, the clinical characteristics and risk of cardiovascular and cerebrovascular (CCV) events in PA patients with aldosterone responsiveness to ACTH stimulation remain unclear. This study aimed to investigate the prevalence of CCV events in PA patients with high aldosterone responsiveness to ACTH stimulation.

Article Synopsis
  • - The study investigates the role of 11β-HSD1, an enzyme linked to metabolic syndrome, focusing on how its gene (HSD11B1) is regulated by epigenetic factors in adipose tissue from patients with different adrenal conditions, including primary aldosteronism (PA) and subclinical Cushing's syndrome (SCS).
  • - mRNA analysis showed that HSD11B1 expression was significantly higher in patients with aldosterone-producing adenoma (APA) and SCS compared to those with non-functioning adrenal adenoma (NFA) or Cushing's adenoma (Cu), indicating potential differences in metabolic impact.
  • - The research also found lower levels of methylation in the HSD

  • Adrenal venous sampling (AVS) is the best method for determining specific types of primary aldosteronism (PA) and measuring aldosterone and cortisol, which can fluctuate throughout the day.
  • A study involving 753 patients in Japan analyzed whether the timing of AVS (morning vs afternoon) affects results related to PA subtype diagnosis and hormone levels.
  • Results showed no major differences in success and diagnostic rates between morning and afternoon samples, but patients with bilateral PA had significantly higher aldosterone levels in the morning compared to the afternoon, suggesting distinct hormonal patterns for different PA subtypes.

Primary aldosteronism (PA) is associated with higher cardiovascular morbidity and mortality rates than essential hypertension. The Japan Endocrine Society (JES) has developed an updated guideline for PA, based on the evidence, especially from Japan. We should preferentially screen hypertensive patients with a high prevalence of PA with aldosterone to renin ratio ≥200 and plasma aldosterone concentrations (PAC) ≥60 pg/mL as a cut-off of positive results.

Background: Aldosterone synthase gene, CYP11B2 is regulated by potassium and angiotensin II (Ang II). We have reported that Ang II could change the DNA methylation status around transcription factor-binding sites and a transcription start site (TSS) and activate expression of CYP11B2. Similar to Ang II, small increases in extracellular potassium levels also increase CYP11B2 mRNA levels.
