Primary neuroendocrine carcinoma (NEC) of the anal canal is a rare, highly malignant tumor with a poor prognosis. Despite the standard first-line treatment with etoposide or irinotecan combined with cisplatin, effective second-line therapies are lacking. In 2019, Japan approved cancer genome profiling (CGP) tests for solid tumors to enhance genomic understanding.

Article Synopsis
  • Gossypiboma is a rare surgical complication where gauze is unintentionally left inside the body, leading to inflammation and tissue adhesion, often without symptoms for years.
  • * It can be difficult to diagnose preoperatively, as common imaging techniques like ultrasound and CT may not clearly show it, especially when located near the gastrointestinal tract.
  • * The report highlights how endoscopic ultrasound and fine-needle aspiration can be effective for diagnosing gossypiboma before surgery by providing detailed imaging and tissue analysis.

An 83-year-old male underwent three transgastric punctures with endoscopic ultrasound-guided fine-needle aspiration for the examination of a pancreatic body tumor. After a diagnosis of resectable pancreatic cancer and undergoing distal pancreatectomy, the patient was administered postoperative adjuvant chemotherapy with oral S-1 for 6 months, and carcinoembryonic antigen and carbohydrate antigen 19-9 levels were bimonthly evaluated. Carbohydrate antigen 19-9 levels continually increased to 4638.

Article Synopsis
  • V-ATPase plays a role in cancer development, and proton pump inhibitors (PPIs) could improve chemotherapy effectiveness for esophageal cancer patients.
  • The study tested three PPIs (lansoprazole, esomeprazole, vonoprazan) alongside 5-fluorouracil (5-FU) on esophageal cancer cells and analyzed outcomes in 40 patients who received 5-FU.
  • Results showed that PPIs significantly boosted the effectiveness of 5-FU in lab tests, and PPI-treated patients had better overall survival rates compared to those without PPI treatment.

Cancer cachexia and the associated skeletal muscle wasting are considered poor prognostic factors, although effective treatment has not yet been established. Recent studies have indicated that the pathogenesis of skeletal muscle loss may involve dysbiosis of the gut microbiota and the accompanying chronic inflammation or altered metabolism. In this study, we evaluated the possible effects of modifying the gut microenvironment with partially hydrolyzed guar gum (PHGG), a soluble dietary fiber, on cancer-related muscle wasting and its mechanism using a colon-26 murine cachexia model.

Epithelial‑mesenchymal transition (EMT) is considered a crucial event in the development of cancer metastasis. Metformin is a drug used in the treatment of type 2 diabetes. Recently, increasing evidence has indicated that metformin possesses anti‑tumor activities.

Article Synopsis
  • Immune checkpoint molecules play a role in regulating tumor immunity by interacting with immune cell ligands, and soluble forms of these molecules have been found in cancer patients' blood.
  • In a study of 47 patients with advanced esophageal cancer, higher levels of soluble PD-1 and CD155 were identified compared to 24 control subjects, indicating a possible link to tumor characteristics and treatment outcomes.
  • The study suggests that soluble CD155 may serve as a positive marker for chemotherapy response and overall survival, while changes in certain soluble molecules like CD226 could indicate poor treatment response, highlighting their potential as biomarkers for esophageal cancer.

It has been reported that docetaxel, cisplatin, and 5-FU combination chemotherapy(DCF)is effective for advanced esophageal cancer. However, we often encounter severe hematotoxicity during DCF therapy. Here, we retrospectively analyzed the data of patients with advanced esophageal cancer treated with DCF at our department, and assessed the efficacy of pegfilgrastim and the issues associated with its use.
