There is insufficient evidence comparing the outcomes of tacrolimus-based remission induction therapy with infliximab in refractory ulcerative colitis (UC) and evidence regarding optimal strategies after tacrolimus-based remission induction therapy. We conducted a multi-institutional retrospective study of patients with UC treated with tacrolimus or infliximab between January 2010 and March 2019. The proportion of clinical remission at week 8 and cumulative colectomy-free rate were examined using propensity score matching analysis.

Serum leucine-rich alpha-2 glycoprotein (LRG) can monitor disease activities during biologics treatment in patients with inflammatory bowel disease (IBD). It is unclear whether the pretreatment serum LRG level can predict clinical effectiveness including serum trough levels of ustekinumab in patients with IBD. This multicenter prospective cohort study included 184 patients (Crohn's disease, 104; ulcerative colitis, 80) who received ustekinumab (n = 119) or anti-tumor necrosis factor (n = 65) between January 2019 and March 2023.

Article Synopsis
  • The study investigates the occurrence and impact of second primary malignancies (SPMs) in patients who underwent treatment for superficial esophageal squamous cell carcinoma (ESCC).
  • Out of 187 patients monitored over nearly 8 years, there was a 92.6% 5-year survival rate, but 14 patients died from SPMs, particularly from lung cancer, which had a high mortality rate.
  • The research highlights the need for comprehensive screening methods, including both esophagogastroduodenoscopy (EGD) and computed tomography, especially for patients showing certain risk factors like multiple Lugol-voiding lesions and metachronous ESCC.

Background: Despite the progress in endoscopic hemostasis and pharmacological treatment, the mortality rate of peptic ulcer bleeding remains at 5-10%. Rebleeding after peptic ulcer bleeding is believed to be a risk factor for mortality. This study aimed to evaluate whether renal dysfunction is a predictor of rebleeding after endoscopic hemostasis in patients with peptic ulcer bleeding.

Background: Meigs' syndrome is a rare disease characterized by a triad of presentations, including benign ovarian tumor, ascites, and pleural effusion. However, a clinical diagnosis of Meigs' syndrome remains challenging because pleural and ascitic effusions can be common findings in a variety of underlying conditions. Furthermore, these findings can often be misdiagnosed as pleural and peritoneal dissemination caused by potentially malignant tumors, leading to the administration of improper treatment.

Chylothorax has been reported to be caused by accidental injuries in half of all cases in Japan, and < 10% of these cases have been associated with malignant tumors, including lymphoma. Chylothorax is a rare complication of gastric carcinoma. We successfully treated a 58-year-old man with gastric carcinoma, chylothorax, and ascites using a combination of talc pleurodesis and a lipid-limited diet.

Background And Study Aims: Endoscopic submucosal dissection (ESD) has a high en bloc resection rate and is widely performed for large superficial colorectal tumors, but delayed bleeding remains one of the most common complications of colorectal ESD. The aim of the present study was to evaluate the clinical efficacy of prophylactic clip closure of mucosal defects for the prevention of delayed bleeding after colorectal ESD.

Patients And Methods: We enrolled consecutive patients with colorectal lesions between January 2012 and May 2017 in this retrospective study.

Article Synopsis
  • A study was conducted in Japan to explore how environmental factors might influence the seasonality of inflammatory bowel disease (IBD) onset and exacerbations, particularly in young patients.
  • The research involved 1,055 patients (298 with Crohn's disease and 757 with ulcerative colitis) and revealed that Crohn's patients were more likely to experience symptoms in summer, while ulcerative colitis did not show seasonal trends.
  • Over 51% of IBD patients reported seasonal exacerbations, primarily during winter, and the study found that younger patients (under 40) exhibited more distinct seasonal patterns compared to older patients.

Despite an increase in the number of reports on medical safety interventions conducted by ward-based pharmacists, only a few reports have classified intervention cases in detail. We classified and compared the types of medical safety intervention conducted by ward-based pharmacists since the introduction of their services. The interventions were classified into: cases that were identified by pharmacists or through asking questions about physicians' prescriptions before dispensing medications (active interventions); and those in which pharmacy technicians could contribute to medical safety by receiving inquiries from patients or healthcare providers (passive interventions).

Background: According to the literature, 40%of all oral complications associated with chemotherapy are due to oral mucositis. Moreover, such complications increase the difficulties associated with oral intake, leading to deterioration of the patient's nutritional condition and increasing the risk of systemic infection. Therefore, oral mucositis prevention and proper treatment are very important.

There are 33 human tetraspanin proteins, emerging as key players in malignancy, the immune system, fertilization, cellular signaling, adhesion, morphology, motility, proliferation, and tumor invasion. CD9, a member of the tetraspanin family, associates with and influences a variety of cell-surface molecules. Through these interactions, CD9 modifies multiple cellular events, including adhesion, migration, proliferation, and survival.

At the Municipal Ikeda Hospital, a system in which pharmacists stationed in one ward pharmacy dispense drugs to be administered by injection and injectable preparations delivered to patients' bedsides was introduced in April 2000. This system was aimed at minimizing risks related to injections. Initially, however, on holidays, nurses played the roles of pharmacists in terms of the injections, and there were concerns over a possible rise in the incidence of errors (adverse events/near-misses) related to injections on these days compared with weekdays.

Salmon milt DNA hydrogel beads were synthesized by an inverse suspension polymerization of acrylamide in the continuous phase of cyclohexane. These DNA hydrogel beads in water medium are stable, more than 82% (w/w) of the DNA can be retained in the hydrogel after a sufficient soaking in water. Comparing with normal adsorbents such as activated carbon and alumina, this DNA matrix showed a selective adsorptivity for the dioxin derivatives with planar structure such as dibenzo-p-dioxin (DD), dibenzofuran (DF) and biphenyl (BP).
