Background: Preoperative hepatitis B virus (HBV) DNA level has been shown to correlate with the prognosis of patients with HBV-associated hepatocellular carcinoma (HCC) following liver resection, but its dynamic changes have not been reported. The aim of this longitudinal multicenter retrospective observational study was to describe the trajectory of HBV DNA after R0 liver resection in patients receiving antiviral therapy and to investigate its impact on clinical outcomes.

Methods: This study included patients with HBV-related HCC from nine hospitals in China who received antiviral therapy and R0 hepatectomy between 2015 and 2016.

Background: In computer-aided diagnosis (CAD) studies utilizing multireader multicase (MRMC) designs, missing data might occur when there are instances of misinterpretation or oversight by the reader or problems with measurement techniques. Improper handling of these missing data can lead to bias. However, little research has been conducted on addressing the missing data issue within the MRMC framework.

Article Synopsis
  • The study aimed to evaluate the effectiveness of glucose-lowering drugs (GLDs) combined with insulin in Chinese patients with type 1 diabetes (T1DM).
  • It compared 121 patients using GLDs alongside insulin to 56 patients using only insulin, measuring changes in glycated hemoglobin A1c (HbA1c) and daily insulin dosage.
  • Results showed a significant reduction in HbA1c and daily insulin use in the GLD+insulin group, suggesting that GLDs can enhance glycemic control and decrease insulin needs without causing severe hypoglycemia.

Article Synopsis
  • The study investigates the link between epilepsy and the increased risk of acute myocardial infarction (AMI), revealing a growing prevalence of AMI in epilepsy patients over a decade.
  • A large sample of over 8 million inpatients was analyzed, finding a significant rise in AMI diagnoses among epilepsy patients, from approximately 1,912 to 2,530 per 100,000 hospitalizations between 2008 and 2017.
  • Factors contributing to higher AMI risk in epilepsy patients included older age, atherosclerosis, conduction disorders, cardiomyopathy, and other health issues, with patients having AMI experiencing notably higher inpatient mortality.

The composite dietary antioxidant index (CDAI) is commonly utilized to assess antioxidant intake across diseases, but its association with kidney stones is unclear. We hypothesized that higher CDAI is associated with reduced kidney stone risk. Using National Health and Nutrition Examination Survey 2007-2020 data, we calculated CDAI based on vitamins A, C, E, selenium, zinc, and carotenoids intake in 29,280 adults.

Purpose: Although the adverse effects of atrial fibrillation (AF) on cancers have been well reported, the relationship between the AF and the adverse outcomes in prostate cancer (PC) remains inconclusive. This study aimed to explore the prevalence of AF and evaluate the relationship between AF and clinical outcomes in PC patients.

Methods: Patients diagnosed with PC between 2008 and 2017 were identified from the National Inpatient Sample database.

Background: The optimal proximal margin (PM) length for Siewert II/III adenocarcinoma of the esophagogastric junction (AEJ) remains unclear. This study aimed to determine the optimal PM length using an abdominal approach to guide surgical decision-making.

Methods: A prospective study analyzed 304 consecutive patients diagnosed with Siewert II/III AEJ between January 2019 and December 2021.

Article Synopsis
  • Hepatocellular carcinoma (HCC) with microvascular invasion (MVI) has a high recurrence risk after liver surgery, prompting the exploration of adjuvant immunotherapy like sintilimab.
  • A phase 2 trial involving 198 patients assessed the effectiveness and safety of sintilimab, comparing it to an active surveillance group, with a focus on recurrence-free survival (RFS) and overall survival (OS).
  • Results showed significant improvements in RFS for the sintilimab group (27.7 months) versus active surveillance (15.5 months), with manageable side effects, suggesting promise for immune checkpoint inhibitors in high-risk HCC patients.

Objective: To investigate the correlation between early-stage blood pressure indexes and prognosis in sepsis patients.

Methods: A retrospective cohort study was conducted on the medical records of patients diagnosed with sepsis from 2001 to 2012 in the Medical Information Mart for Intensive Care-III (MIMIC-III) database. Patients were divided into survival group and death group according to the 28-day prognosis.

Background: Achieving a better prognosis for patients and reducing the risk of complications are primary considerations in surgical decisions for hilar cholangiocarcinoma.

Methods: A retrospective analysis of the authors' clinical practice outcomes in the surgical management of patients with hilar cholangiocarcinoma following the planned-hepatectomy surgical treatment programme between 2009 and 2018.

Results: Some 473 patients were included, of whom 127 (26.

Background: Stanford type A aortic dissection (T(A)AD) is one of the most dangerous cardiovascular diseases and morbid obesity is associated with the prognosis of many cardiovascular diseases. The aim of this study is to investigate the impact of morbid obesity on in-hospital mortality, total hospital costs and discover the prevalence of morbid obesity among inpatients with T(A)AD.

Methods: Patients with a primary diagnosis of T(A)AD were identified from the National Inpatient Sample database (NIS) from 2008 to 2017.

Background: In the United States, sepsis accounts for 13% of the total hospital expenses and > 50% of hospital deaths. Moreover, people with sepsis are more likely to be readmitted.

Objective: The aim of this study was to assess the prevalence and outcomes of different hospital readmissions (DHRs) in patients with sepsis, and the factors associated with DHR.

Novel strategies for prostate cancer therapy are required to overcome resistance to abiraterone and enzalutamide. Here, we show that increasing 3βHSD1 after abiraterone and enzalutamide treatment is essential for drug resistance, and biochanin A (BCA), as an inhibitor of 3βHSD1, overcomes drug resistance. 3βHSD1 activity increases in cell lines, biopsy samples, and patients after long-term treatment with enzalutamide or abiraterone.

  • The study compares the efficacy of two treatments for hepatocellular carcinoma (HCC): transarterial chemoembolization (TACE) combined with a PD-1 inhibitor versus TACE combined with a PD-1 inhibitor and lenvatinib.
  • Results indicated that patients receiving the triple therapy had significantly better progression-free survival (PFS) and overall survival (OS) compared to those on the double therapy.
  • While the triple therapy showed better clinical outcomes, it also had common treatment-related adverse events like decreased albumin and platelet count, but these were manageable.

Background: Microvascular invasion (MVI) is a significant risk factor affecting survival outcomes of patients after R0 liver resection (LR) for hepatocellular carcinoma (HCC). The current classification of MVI is not refined enough to prognosticate long-term survival of these patients, and a new MVI classification is needed.

Methods: Patients with HCC who underwent R0 LR at the Eastern Hepatobiliary Surgery Hospital from January 2013 to December 2013 and with resected specimens showing MVI were included in this study with an aim to establish a novel MVI classification.

Article Synopsis
  • Colchicine has been shown to have protective effects on the heart, particularly in treating myocardial ischemia reperfusion injury (MIRI), but its specific mechanisms had not been well-studied until now.
  • Researchers utilized network pharmacology and various experimental methods to identify 138 key target genes that colchicine interacts with to potentially treat MIRI.
  • The study's findings indicate that colchicine exerts its protective effects against cell injury through anti-apoptotic mechanisms, enhancing cell survival by regulating critical proteins involved in apoptosis.
