Acute opioid withdrawal on accidental injection of naltrexone.

We report two 16-year-old female intravenous drug users who, after making purchases from street suppliers, both presented with symptoms of acute opioid withdrawal. Urine toxicology revealed naltrexone, a long-acting opioid antagonist used in detoxification and maintenance therapy in opioid dependence. While the safety and efficacy of opiate antagonist treatment is being debated, the present case highlights the vulnerability of this young population.

A novel RNA polymerase II C-terminal domain phosphatase that preferentially dephosphorylates serine 5.

The transcription and processing of pre-mRNA in eukaryotic cells are regulated in part by reversible phosphorylation of the C-terminal domain of the largest RNA polymerase (RNAP) II subunit. The CTD phosphatase, FCP1, catalyzes the dephosphorylation of RNAP II and is thought to play a major role in polymerase recycling. This study describes a family of small CTD phosphatases (SCPs) that preferentially catalyze the dephosphorylation of Ser5 within the consensus repeat.

Adolescent admissions to a tertiary paediatric hospital: a dynamic pattern.

Introduction: Adolescent inpatient facilities emerged in Australia in the 1980s to cater for an increasing number of young people with chronic illness and disability. Yet, there is minimal published data on the number of young people admitted to hospital within these units, their unit of admission, length of stay or reason for admission. A 12-year audit of adolescents admitted to a tertiary hospital with a dedicated adolescent unit and adolescent medicine programme in Victoria, Australia, was conducted to review the pattern of hospitalisation in young people in order to provide data to assist healthcare policy and planning agendas.

Strategies to promote better outcomes in young people with chronic illnesses.

Introduction: This paper aims to provide an overview of chronic illness and disability in young people, focusing on the developmental needs of young people and the impact of chronic illness on adolescence. The emotional well-being of the young person with chronic illness is explored and strategies that promote better health outcomes for the young person through health, family and school settings are discussed.

Methods: Literature was searched using Medline from 1980 to 2002 using the keywords chronic illness, chronic disease, disability, psychological, social, outcomes and transition.

Structural basis for recruitment of glycogen synthase kinase 3beta to the axin-APC scaffold complex.

Glycogen synthase kinase 3beta (GSK3beta) is a serine/threonine kinase involved in insulin, growth factor and Wnt signalling. In Wnt signalling, GSK3beta is recruited to a multiprotein complex via interaction with axin, where it hyperphosphorylates beta-catenin, marking it for ubiquitylation and destruction. We have now determined the crystal structure of GSK3beta in complex with a minimal GSK3beta-binding segment of axin, at 2.

The regulation of glycogen synthase kinase-3 nuclear export by Frat/GBP.

Previous studies have shown that nuclear levels of glycogen synthase kinase-3 (GSK-3) are dynamically regulated and may affect access of GSK-3 to its substrates. In this study we show that the GSK-3-binding protein Frat/GBP regulates the nuclear export of GSK-3. We show that Frat/GBP contains a nuclear export sequence that promotes its own nuclear export and that of associated GSK-3.

Distance health services under the general agreement on trade in services.

This Article discusses the state of distance health with respect to the General Agreement on Trade in Services (GATS). After examining the various aspects of telemedicine and its place in international trade, the author looks at the structure and functioning of GATS and how telemedicine is regulated under this agreement. The author argues that the potential for telemedicine under this agreement has yet to be fulfilled and suggests a number of ways to realize its potential.

Identification of the Axin and Frat binding region of glycogen synthase kinase-3.

Glycogen synthase kinase-3 (GSK-3) is a key component of several signaling pathways including those regulated by Wnt and insulin ligands. Specificity in GSK-3 signaling is thought to involve interactions with scaffold proteins that localize GSK-3 regulators and substrates. This report shows that GSK-3 forms a low affinity homodimer that is disrupted by binding to Axin and Frat.

Membrane blebbing during apoptosis results from caspase-mediated activation of ROCK I.

The execution phase of apoptosis is characterized by marked changes in cell morphology that include contraction and membrane blebbing. The actin-myosin system has been proposed to be the source of contractile force that drives bleb formation, although the biochemical pathway that promotes actin-myosin contractility during apoptosis has not been identified. Here we show that the Rho effector protein ROCK I, which contributes to phosphorylation of myosin light-chains, myosin ATPase activity and coupling of actin-myosin filaments to the plasma membrane, is cleaved during apoptosis to generate a truncated active form.

Antibiotic sensitivity of Propionibacterium acnes isolates from patients with acne vulgaris in a tertiary dermatological referral centre in Singapore.

Objectives: To study the minimum inhibitory concentrations (MICs) of Propionibacterium acnes (P. acnes) isolates to selected antibiotics from patients with acne vulgaris in Singapore and determine if resistance increases with prolonged use of antibiotics.

Design: A single-centre prospective study.

Chronic peritoneal inflammation by cyanate in rats.

Objective: During peritoneal dialysis, the peritoneum is exposed to waste products, including urea. Urea forms cyanate spontaneously at body temperature and pH, and cyanate carbamylates amino acids, peptides, and proteins. Cyanate may contribute to peritoneal injury with morphological changes in the peritoneum.

The skeletal response to matt and polished cemented femoral stems.

We studied the effect of the surface finish of the stem on the transfer of load in the proximal femur in a sheep model of cemented hip arthroplasty. Strain-gauge analysis and corresponding finite-element (FE) analysis were performed to assess the effect of friction and creep at the cement-stem interface. No difference was seen between the matt and polished stems.

Substance use by indigenous and non-indigenous primary school students.

Objective: Recent Australian research with adolescents aged 13 to 17 years has found that Indigenous youth are more likely than non-Indigenous adolescents to smoke tobacco and cannabis, although they may be less likely to use alcohol. The objective of this study was to examine whether this pattern exists among younger children.

Method: A school-based, self-report survey was conducted in primary schools that had high proportions of Aboriginal and Torres Strait Islander children.

Staphylococcus lugdunensis: report of first case of skin and soft tissue infection in Singapore.

We report the first case of skin and soft tissue infection due to Staphylococcus lugdunensis in Singapore. This is a coagulase negative Staphylococcus species known to cause a wide variety of more serious infections--brain abscess, sepsis, chronic osteomyelitis and infective endocarditis.

Lipid peroxidation in small and large phospholipid unilamellar vesicles induced by water-soluble free radical sources.

The susceptibility of small and large egg yolk phosphatidylcholine unilamellar vesicles to Fe(2+)/histidine-Fe(3+)- and Fenton reagent (Fe(2+)-H(2)O(2))-induced lipid peroxidation was evaluated by measuring the formation of thiobarbituric acid reactive substances (TBARS). It has been found that surface curvature or phospholipid packing exerts significant effect on the oxidative susceptibility of the unsaturated lipid bilayers and the highly curved and loosely packed small unilamellar vesicles (SUVs) exhibit much less resistance to the oxidative stress induced by the water-soluble free radical sources. The presence of lipid hydroperoxides in sonicated vesicles was excluded as the cause for higher level of lipid peroxidation in the phospholipid SUVs.

Role of caspase-3 in apoptosis of colon cancer cells induced by nonsteroidal anti-inflammatory drugs.

Epidemiological studies have demonstrated that nonsteroidal anti-inflammatory drugs (NSAIDs) decrease the incidence of and mortality from colon cancer. In addition, NSAIDs reduce the number and the size of polyps in patients with familial adenomatous polyposis. The mechanisms responsible for the antineoplastic effect of NSAIDs are not yet completely understood, but one of the possible mechanisms is an induction of apoptosis.

Alternative splicing of the neurotrophin-3 gene gives rise to different transcripts in a number of human and rat tissues.

The mouse neurotrophin-3 (NT-3) gene has been shown to contain two exons (exon 1A and exon 1B) upstream of the single coding exon (exon 2). These upstream exons are alternatively spliced to the coding exon, generating two different NT-3 transcripts. We investigated whether alternative splicing of two upstream exons also occurs in the human and rat NT-3 gene.

Autologous transfusion in obstetrics.

Aim Of Study: To assess the safety of blood collection for autologous transfusion in pregnant women and the effects of the procedure on the fetus.

Method: Prospective study involving obstetric patients undergoing elective lower segment Caesarean section (LSCS) between 1993 and 1994 were recruited for autologous transfusion. Continuous cardiotocograph monitoring was carried out throughout the duration of the procedure and the fetal haemodynamics was studied before and after by doppler blood flow ultrasound.

Predictors of hysterectomy: an Australian study.

Objective: We evaluated the relative importance of predictors of hysterectomy.

Study Design: A questionnaire survey of an Australia-wide sample of adult female twins was conducted; self-report data were validated against reports from treating physicians.

Results: A total of 3096 women (94%) and 366 physicians (87%) responded.
