Background: Screening for frequent hospitalizations in the community can help prevent super-utilizers from growing in the inpatient population. However, the determinants of frequent hospitalizations have not been systematically examined, their operational definitions have been inconsistent, and screening among community members lacks tools. Nor do we know if what determined frequent hospitalizations before COVID-19 continued to be the determinant of frequent hospitalizations at the height of the pandemic.

Importance: Recent evolutions in clinical care and remote monitoring suggest that some acute illnesses no longer require intravenous therapy and inpatient hospitalization.

Objective: To describe outcomes of patients receiving care in a new, outpatient, virtual, home-based acute care model called Safer@Home.

Design, Setting, And Participants: This retrospective cohort analysis, conducted from September 1, 2022, through August 31, 2023, included 2466 patients treated at a safety net hospital in Los Angeles County for 10 core illnesses and 24 other acute illnesses for which patients are commonly hospitalized.

Maternal infections during pregnancy can increase the risk to offspring of developing a neurodevelopmental disorder. Given the global prevalence and severity of infection with Severe Acute Respiratory Syndrome related Coronavirus 2 (SARS-CoV-2), the objective of this study was to determine if in utero exposure to severe maternal SARS-CoV-2 infection alters infant neurodevelopmental outcomes at 12 months and to identify potential biological markers of adverse infant outcomes. Mother-infant dyads exposed to severe SARS-CoV-2 infection (requiring hospitalization) during pregnancy and age and sociodemographic matched control dyads were recruited from Monash Medical Centre, Australia in 2021/22 and prospectively assessed over 12 months.

The Plan-Do-Study Act (PDSA) ramp is a framework that uses initial small changes to build consensus and momentum for subsequent, iterative process improvement. Pain interferes with one's work and social life and, at a personal level, daily activities, mood, and sleep quality. However, little research has been conducted on pain interference and its socioecological determinants among the working poor. Noting the clinical/policy decision needs and the technical challenges of isolating the intricately interrelated socioecological factors' unique contributions to pain interference and quantifying the relative contributions of each factor in an interpretable manner to inform clinical and policy decision-making, we deployed a novel random forest algorithm to model and quantify the unique contribution of a diverse ensemble of environmental, sociodemographic, and clinical factors to pain interference.

While much is known about governance models for research informatics programs in academic medical centers and similarly situated cancer centers, community and public health systems have been less well-characterized. Infections with endoparasites, especially gastrointestinal helminths, are a common finding in client-owned dogs. The Community Practice section at the Ohio State University Veterinary Medical Center (OSU-VMC) follows Companion Animal Parasite Council, American Animal Hospital Association, and American Veterinary Medical Association guidelines for parasitology by recommending annual fecal analyses for dogs and prescribing year-round, broad-spectrum parasite preventatives. There is increasing interest in determining if parasite occurrence is changing in client-owned dogs.
