Paediatric regimens which incorporate asparaginase have improved outcomes of adolescents and younger adults with acute lymphoblastic leukaemia but are limited by toxicity. This retrospective report by Tinajero and Xu suggests that delaying the timing of asparaginase administration during ALL induction may reduce risk of hepatotoxicity. Commentary on: Tinajero et al.

Article Synopsis
  • - Immune effector cell-associated hematotoxicity (ICAHT) is a common toxicity linked to CAR T-cell therapy, notably affecting patients with various lymphomas and multiple myeloma, but has not been previously studied in B-cell acute lymphoblastic leukemia (B-ALL).
  • - In a study involving 156 young patients with relapsed/refractory B-ALL, severe neutropenia was observed for a median of 13 days, with over half experiencing significant ICAHT, which indicated the need for better predictive models for this condition.
  • - The researchers developed the ALL-Hematotox (ALL-HT) score, improving on existing models by incorporating bone marrow disease burden, which accurately predicted severe neutrop

Article Synopsis
  • - Asparaginase-containing regimens for acute lymphoblastic leukemia (ALL) result in a significant prevalence of venous thromboembolism (VTE) among adolescents and young adults, with 1-year and 2-year cumulative incidences of 31.9% and 33.5%, respectively, particularly during the ASP-based consolidation phase.
  • - The study revealed that overweight or obese patients had a higher risk of developing VTE (39.2%) compared to those with a normal BMI (29.0%), and overall survival rates were similar regardless of VTE occurrence, at around 91.5%.
  • - Despite the frequent occurrence of VTE, especially types like pulmonary embolism and deep vein thrombosis,

Chimeric antigen receptor (CAR) T-cell therapy has transformed the care of lymphoma, yet many patients relapse. Several prognostic markers have been associated with CAR T-cell outcomes, such as tumor burden, response to bridging chemotherapy, and laboratory parameters at the time of lymphodepletion or infusion. The effect of cancer cachexia and weight loss before CAR T cells on toxicity and outcomes is not well understood.

Purpose: On the basis of the results of the ZUMA-3 trial, brexucabtagene autoleucel (brexu-cel), a CD19-directed chimeric antigen receptor T-cell therapy, gained US Food and Drug Administration approval in October 2021 for adults with relapsed/refractory (R/R) B-cell ALL (B-ALL). We report outcomes of patients treated with brexu-cel as a standard therapy.

Methods: We developed a collaboration across 31 US centers to study adults with B-ALL who received brexu-cel outside the context of a clinical trial.

  • Venetoclax, a BCL2 inhibitor for chronic lymphocytic leukemia (CLL), can induce tumor lysis syndrome (TLS), prompting a study to assess its incidence and risk factors among patients treated between 2016 and 2020.
  • Out of 136 patients with 616 medication escalations, 5.1% developed laboratory TLS, while none experienced clinical TLS, with a higher incidence seen in inpatient escalations (15%) compared to outpatient (2.7%).
  • Prophylactic measures, such as intravenous hydration and medication like allopurinol, were commonly used and likely contributed to the low rate of TLS, especially in outpatient settings.

The effect of prior inotuzumab ozogamicin (InO) treatment on brexucabtagene autoleucel (brexu-cel) outcomes remains unclear in adults with acute lymphoblastic leukemia (ALL). We conducted a retrospective multicenter analysis of 189 patients with relapsed/refractory ALL treated with brexu-cel. Over half of the patients received InO before brexu-cel (InO exposed).

Article Synopsis
  • - AYA patients with acute lymphoblastic leukemia (ALL) have poorer treatment completion rates compared to children, with only 60.8% of AYA patients completing their treatment versus 89.7% of children.
  • - The main reason for this discrepancy is the higher incidence of early treatment failure among AYA patients (14.5% vs. 2.4%), while withdrawals due to toxicity or personal issues are less common but still higher in AYA patients (9.3% vs. 4.7%).
  • - Staying on treatment for at least one year significantly improves survival rates, with AYA patients showing 5-year overall survival of 88.9%, but they completed critical asparaginase

Adolescent and young adults (AYAs) with acute lymphoblastic leukemia (ALL) treated with asparaginase-containing pediatric regimens are commonly overweight or obese. We studied the association of body mass index (BMI) on outcomes of 388 AYAs aged 15 to 50 years treated on Dana-Farber Cancer Institute (DFCI) consortium regimens (2008-2021). BMI was normal in 207 (53.

Nelarabine, an antimetabolite prodrug, is approved as monotherapy for children and adults with relapsed and refractory T-cell acute lymphoblastic leukemia and lymphoma (R/R T-ALL/LBL), although it is often used in combination regimens. We sought to understand differences in efficacy and toxicity when nelarabine is administered alone or in combination. We retrospectively analyzed 44 consecutive patients with R/R T-ALL/LBL; 29 of whom were treated with combination therapy, most with cyclophosphamide and etoposide (23, 79%) and 15 with monotherapy.

Osteonecrosis (ON) is a complication of acute lymphoblastic leukaemia (ALL) treatment with patient- (age, female sex, genetic polymorphisms, presence of metabolic syndrome) and treatment-specific (glucocorticoid type and schedule) risk factors described. The potential role of asparaginase in increasing risk of ON via effects on coagulation, lipid metabolism, and steroid clearance is now also recognised. Paediatric studies consistently identify age as a key risk factor for ON, with adolescents at higher risk than young children.

We present the case of a 61-year-old woman who presented with acutely worsening right upper quadrant pain and was found to be in acute liver failure with Klebsiella pneumoniae bacteremia. Despite aggressive intensive care management, the patient ultimately died of refractory shock attributed to sepsis and fulminant liver failure. On autopsy, she was found unexpectedly to have diffuse intrahepatic cholangiocarcinoma with metastases to regional lymph nodes and intravascular spread to the lungs.

Adolescent and young adult patients with acute lymphoblastic leukemia (ALL) have superior outcomes when treated on pediatric regimens. Pediatric ALL regimens rely heavily on corticosteroids and asparaginase and are known to increase the risk of osteonecrosis (ON) and fractures in children, particularly adolescents. Orthopedic toxicity among young adults treated on pediatric-inspired regimens is not well described.
