Solid and cystic tumor of the pancreas: clinicopathologic and genetic studies of four cases.

Background: Solid and cystic tumor (SCT) of the pancreas can be distinguished from other pancreatic neoplasms by its nearly exclusive occurrence in young women, and its favorable prognosis after complete resection.

Methods: We experienced four cases with SCT of the pancreas, and analyzed these tumors by immunohistochemical and electron microscopic studies, as well as genetic analysis of ras oncogene mutation.

Results: The presented cases expressed the neuron-specific enolase in two cases, alpha1-antitrypsin and alpha1-antichymotrypsin in two cases, and vimentin in one case, which indicated that this tumor originates from pleuripotential embryonic stem cells.

Improved liver function following infusion of fructose-1, 6-bisphosphate in posthepatectomy patients.

The clinical effect of fructose-1,6-bisphosphate (FBP) administered to posthepatectomy patients was examined. FBP at 0.25 mmol/kg was administered continuously into the hepatic artery for 60 minutes on the 1st postoperative day in 11 cases.

Laparoscope-assisted Billroth I gastrectomy.

We performed a distal gastrectomy, including regional lymph node dissection, under laparoscopic observation followed by Billroth I. (B-I) anastomosis using autosuture in five patients with early gastric cancer. We had previously evaluated the technique for reconstructive surgery in animal experiments.

Total parenteral nutrition in posthepatectomy patients.

Blood biochemical and nutritional metabolism indices were examined in eight patients who received infusion containing glucose, fructose, and xylitol in a 4:2:1 ratio (group GFX) after liver resection compared with those in six patients who received only glucose (group G). Preoperative patient-selection criteria consisted of a parabolic oral glucose tolerance test level over time, a total activity of coagulation factors II, VII, and X of > or = 60%, and an indocyanine green disappearance rate (ICG K) of > or = 0.13.

Beneficial effects of fructose-1,6-diphosphate infusion on liver regeneration after ischemic liver injury.

The effect of fructose-1,6-diphosphate (FDP) on cellular viability after partial hepatectomy in partial ischemic liver was investigated in rats. The administration of FDP did not increase blood flow in the hepatic tissue; however, it significantly suppressed the elevation of serum liver functions for 24 hours after partial hepatectomy. [Beneficial effects of fructose-1,6-diphosphate infusion on hepatic regeneration after partial hepatectomy by microwave tissue coagulator].

Experiments were performed to explore the effect of microwave tissue coagulator (MTC) on post-lobectomy regeneration of the liver and the regeneration-promoting effect of fructose-1,6-diphosphate (FDP) on heat injury of the liver after 40% hepatectomy in rats. Concerning the effect of MTC on hepatic regeneration, a MTC group showed a significantly higher serum LDH level on post-operative day 2, compared with a simple ligation group. A significant difference in DNA synthesizing activity between two groups was noted only in the peripheral region of regenerating liver tissue.

[A clinicopathological study on carcinoembryonic antigen in breast cancer].

The production of carcinoembryonic antigen (CEA) by human breast cancer tissue has been studied in relation to the prognosis of 60 patients with breast cancer who were classified in Clinical Stage of UICC I, II and III, and who were treated by radical mastectomy. Tissue CEA was studied in primary tumors using an immunoperoxidase (PAP) method. The obtained results were as followings: 1) Patients who had CEA-negative tumors had significantly higher 10-year survival rates.

[A case of stromal sarcoma of the breast].

A 28-year-old woman with a stromal sarcoma of the breast is reported. By palpation, she had uncovered a tumor in the upper inner quadrant of the right breast. The subsequent physical examination and echographic findings suggested a phyllodes tumor and an excisional biopsy was performed.

Although many patients with humoral hypercalcemia of malignancy exhibit reduced serum 1,25-dihydroxyvitamin D [1,25-(OH)2D] levels, N-terminal fragments of recently identified PTH-related protein as well as PTH itself elevate serum 1,25-(OH)2D concentrations. In the present study, the effect of tumor extracts from human tumor-implanted hypercalcemic nude rat models with high and low serum 1,25-(OH)2D on renal 1,25-(OH)2D3 production was examined using rat kidney cells in culture. Whereas tumors from rats with high serum 1,25-(OH)2D levels (OCC rats) contained only a single peak of cAMP production-stimulating activity (CPSA) in osteogenic sarcoma cells on reverse phase HPLC, tumor extracts from rats with low serum 1,25-(OH)2D levels (UCC rats) contained at least two peaks of CPSA.

[Necessity of adjuvant therapy after radical mastectomy based on a study of patients with negative nodes, who have recurrent breast cancer].

The status of the regional lymph nodes influences the prognosis of women with breast cancer who have received a radical mastectomy. To investigate the characteristics of patients with negative nodes who have had a recurrent breast cancer, a statistical study has been carried out involving 279 women who had received a radical mastectomy in our hospital from July 1970 to October 1986. From this study we conclude that systemic adjuvant therapy after radical mastectomy is necessary for patients with negative nodes who have had a breast cancer greater than 2.

[A study on the definition of early breast cancer].

The most important factor in improving the survival rates of the patients with breast cancer is early detection. A high cure rate can be expected if the condition is discovered in early stage. There were 195 patients with primary breast cancer in our hospital from August 1978 to November 1986.

Postoperative intravenous administration of lysine acetylsalicylate: effect on pain relief and platelet function.

The effects of lysine acetylsalicylate, a new injectable salicylate, on postoperative pain relief and platelet function were studied in ten men who had surgery for peptic ulcer. Four of five patients receiving lysine acetylsalicylate had satisfactory pain relief. One patient required an additional injection of 15 mg of pentazocine.

[The influence of a biological response modifier on thoracic lymphocytes--a study involving clinical cases and animal experiments].

The influence of a biological response modifier on thoracic lymphocytes was studied clinically and experimentally. The thoracic lymphocytes and peripheral blood were taken from nine cases of gastric cancer relapse before and after the administration of hemolytic streptococci, OK-432 (PIC), for immunological examination. T: B cell ratio, T cell ratio and ADCC activity of the thoracic lymphocytes and peripheral blood were not changed but the NK activities of both were increased.
