Background: Status epilepticus (SE) has traditionally been thought to cause cerebrospinal fluid (CSF) pleocytosis. However, attributing CSF pleocytosis solely to SE without addressing the underlying etiology may lead to poor outcomes. Leukocyte recruitment to CSF has been shown to peak around 24 hours after prolonged seizures in animal studies, suggesting that CSF pleocytosis within the first 24 hours of SE onset may be due to underlying causes.

Piracetam, a derivative of gamma-aminobutyric acid (GABA), has been used extensively for treatment of myoclonus in a variety of conditions, but not in patients with idiopathic generalized epilepsy (IGE). We have treated a patient with juvenile myoclonic epilepsy who had frequent and inconvenient morning myoclonus with 3,200 mg of piracetam daily. She had had only two generalized tonic-clonic seizures, with the last seizure 10 years earlier.

Combining electroencephalogram (EEG) and functional MRI (fMRI) allows localization of brain regions activated as a result of epileptic spikes. The statistical analysis of fMRI data usually includes a standard model of the hemodynamic response function (HRF) but it is not known how well this fits the actual HRF of epileptic spikes. The objective of this exploratory study was to compare the activated areas and t-statistical scores obtained with a standard HRF to those obtained with a patient-specific HRF.
