The birth of rhinoplasty in the West was driven by a great demand for reconstruction of noses destroyed voluntarily or not, post-traumatic or medical. Two men emerge strongly through their work and writings: Gaspare Tagliacozzi for the brachial flap and Joseph Carpue for the frontal flap. Both of them describe a different method but based on the same skin flap principles.

Goal: Of the first nasal tip modeling procedures performed, the Anderson tripod concept is the most well known, and many anatomy-related studies have analyzed the components involved in tip surgery. The main goals of nasal tip surgery are to stabilize the tip complex, which largely affects the shape, projection and rotation of the nasal tip.
Objective: The present study describes a new, original method, the double columellar strut and its main advantages compared to conventional techniques used for structural rhinoplasty.
Suture techniques are widely used and can be very useful and efficient to support the nasal tip and correct its deformities. Nevertheless, they are so powerful that they have to be performed very precisely if the surgeon does not want to create cosmetics as well as functional problems. The authors describe the main tip sutures and their effects, expected or not.

Main indications are correction of deviated nose, narrowing of bony vault and roofing of an open roof after important hump resection. The nose should be very precisely analyzed before osteotomies if the surgeon doesn't want to create deformities such as step, inverted V, pinched nose or saddle nose.
