Purpose: The focus of this article is to describe the configuration, testing, and commissioning of a novel gantry-less synchrotron-based proton therapy (PT) facility.

Materials And Methods: The described PT system delivers protons with a water equivalent range between 4 and 38 cm in 1800 energy layers. The fixed beam delivery permits a maximum field size of 28 × 30 cm.

Article Synopsis
  • The study aimed to assess how consistently patients are positioned during upright treatment for head and neck cancers using image-guided adaptive proton therapy (IGAPT).
  • Ten patients underwent treatment with a gantry-less system, using various imaging techniques to monitor and adjust their positioning prior to and after irradiation.
  • Results showed that the gantry-less system provided reliable patient positioning, though a small vertical shift was noted that requires further investigation in upcoming studies focusing on thoracic treatments.

An Addendum to the AAPM's TG-51 protocol for the determination of absorbed dose to water is presented for electron beams with energies between 4 MeV and 22 MeV ( ). This updated formalism allows simplified calibration procedures, including the use of calibrated cylindrical ionization chambers in all electron beams without the use of a gradient correction. New data are provided for electron beams based on Monte Carlo simulations.

Background: Adaptive radiation therapy (ART) offers a dynamic approach to address structural and spatial changes that occur during radiotherapy (RT) for locally advanced head and neck cancers. The integration of daily ART with Cone-Beam CT (CBCT) imaging presents a solution to enhance the therapeutic ratio by addressing inter-fractional changes.

Methods: We evaluated the initial clinical experience of daily ART for patients with head and neck cancer using an online adaptive platform with intelligence-assisted workflows on daily CBCT.
