A variety of dearomatized compounds have been prepared in moderate to excellent yields from planar scaffolds using trichloroisocyanuric acid (TCCA) as an atom-economical chlorinating agent. The method tolerates a broad range of functionalities and can take place in several green and/or sustainable solvents. Twenty-one examples of 1,1-dichlorinated products of dearomatized 2-naphthols and analogous heteroarenes (quinolinols, isoquinolinols, and quinazolinol) are reported along with five examples of monochlorinated dearomatized products.

Background: Days at home after surgery is a promising new patient-centred outcome metric that measures time spent outside of healthcare institutions and mortality. The aim of this scoping review was to synthesize the use of days at home in perioperative research and evaluate how it has been termed, defined, and validated, with a view to inform future use.

Methods: The search was run on MEDLINE, Embase, and Scopus on 30 March 2023 to capture all perioperative research where days at home or equivalent was measured.

An essential aspect of the EU's "Registration, Evaluation, Authorisation and Restriction of Chemicals" (REACH) regulation is the European Chemicals Agency's (ECHA) evaluation of testing proposals submitted by registrants to address data gaps. Registrants may propose adaptations, such as read-across, to waive standard testing; however, it is widely believed that ECHA often finds justifications for read-across hypotheses inadequate. From 2008 to August 2023, 2,630 testing proposals were submitted to ECHA; of these, 1,538 had published decisions that were systematically evaluated in this study.

Background: In 2007, Cancer Care Ontario created Thoracic Surgical Oncology Standards for the delivery of surgery, including lobectomy, esophagectomy, and pneumonectomy. These standards regionalized thoracic surgery into designated centers and mandated physical and human resources. This analysis sought to identify the impact of these standards, hereafter referred to as "regionalization," on outcomes after thoracic oncology surgery in Ontario, Canada.

The clinical case of a 53-year-old woman diagnosed with a right Bochdalek hernia complicated by intestinal obstruction is presented. This type of diaphragmatic hernia is rare, especially those located on the right side, but is the most common non-hiatal hernia. Her diagnosis focused on the computed tomography of the chest and abdomen and the repair was performed using a laparoscopic approach.

  • The study looked at how a video game called Bootle Blast helps kids with cerebral palsy learn to improve their movement skills during therapy sessions.
  • Five kids played Bootle Blast while therapists watched and rated how the game and therapists taught different motor skills.
  • The results showed that the game makes therapy feel like fun, but sometimes it doesn’t work perfectly for everyone and needs some improvements.

Article Synopsis
  • The study aimed to analyze the relationship between the sex of surgeons and the number of days patients spend alive and at home (DAH) after surgery.
  • It found that patients treated by female surgeons had significantly more DAH compared to those treated by male surgeons over all measured time frames (30, 90, and 365 days).
  • The findings suggest that higher DAH associated with female surgeons could lead to lower healthcare costs and better patient quality of life, indicating a need for further research in different healthcare settings.

An important element of the European Union's "Registration, Evaluation, Authorisation and Restriction of Chemicals" (REACH) regulation is the evaluation by the European Chemicals Agency (ECHA) of testing proposals submitted by the registrants to address data gaps in standard REACH information requirements. The registrants may propose adaptations, and ECHA evaluates the reasoning and issues a written decision. Read-across is a common adaptation type, yet it is widely assumed that ECHA often does not agree that the justifications are adequate to waive standard testing requirements.

Article Synopsis
  • Some children with cerebral palsy (CP) in underserved areas have trouble getting therapy, so a study was done using a fun video game called Bootle Blast to help them practice their arm movements at home for 8 weeks.
  • The kids set weekly goals for playing and improving daily activities, while researchers measured their progress with different tests before and after the game sessions.
  • Out of 15 kids who started, 13 finished the program, and many showed improvements in their arm movement skills, with most enjoying the game and spending lots of time playing it.

Article Synopsis
  • Airway management is crucial for critically ill trauma patients, and the study examines the use of emergency surgical airway (ESA) as a backup method when endotracheal intubation (ETI) fails.
  • The research analyzed data from 2017 to 2022, finding that out of over 6 million cases, 2264 ESAs were performed, primarily in patients with more severe injuries, especially to the head/neck and face.
  • The study showed higher survival rates for ETI-only patients over those who received an ESA, and specific types of injuries (like firearms and stabbings) increased the likelihood of needing an ESA.

Per- and poly-fluoroalkyl substances (PFAS) are emerging contaminants of concern because of their wide use, persistence, and potential to be hazardous to both humans and the environment. Several PFAS have been designated as substances of concern; however, most PFAS in commerce lack toxicology and exposure data to evaluate their potential hazards and risks. Cardiotoxicity has been identified as a likely human health concern, and cell-based assays are the most sensible approach for screening and prioritization of PFAS.

Despite consensus guidelines, most patients with early-stage triple-negative (TN) and HER2-positive (HER2+) breast cancer do not see a medical oncologist prior to surgery and do not receive neoadjuvant chemotherapy (NAC). To understand barriers to care, we aimed to characterize the relationship between geography (region of residence and cancer centre proximity) and receipt of a pre-treatment medical oncology consultation and NAC for patients with TN and HER2+ breast cancer. Using linked administrative datasets in Ontario, Canada, we performed a retrospective population-based analysis of women diagnosed with stage I-III TN or HER2+ breast cancer from 2012 to 2020.

Article Synopsis
  • Hemorrhage is a major cause of death, and blood products, particularly low titer group O whole blood (LTOWB), are increasingly used to treat patients with hemorrhagic shock.
  • A study analyzed data from over 12,000 trauma patients, finding that 30% received LTOWB within the first 4 hours after hospital arrival.
  • Results indicated that while LTOWB did not show a survival difference at 6 hours, patients receiving at least 10% of LTOWB relative to total blood products had a higher chance of surviving for 24 hours.
