This paper introduces a novel unsupervised inverse-consistent diffeomorphic registration network termed IConDiffNet, which incorporates an energy constraint that minimizes the total energy expended during the deformation process. The IConDiffNet architecture consists of two symmetric paths, each employing multiple recursive cascaded updating blocks (neural networks) to handle different virtual time steps parameterizing the path from the initial undeformed image to the final deformation. These blocks estimate velocities corresponding to specific time steps, generating a series of smooth time-dependent velocity vector fields.

Duration, course and caregiver burden of croup in children: two observational cohorts.

Objectives: Although croup is a common respiratory illness, there is little published regarding symptom course. We aimed to assess symptom progression and caregiver burden, and whether age, sex or season and initial severity of disease are associated with symptom duration.

Design, Setting And Participants: We conducted a secondary analysis of two Canadian prospective cohorts of children 0-16 years old diagnosed with croup; one recruited from a paediatric emergency department (ED) (307 children) between November 1999 and March 2000, and the other from 26 general EDs (1214 children) between September 2002 and April 2006.

The impact of obstructive sleep apnoea on post-operative outcomes.

Background: Unrecognised obstructive sleep apnoea (OSA) has been associated with adverse cardiorespiratory perioperative outcomes. However, with changing anaesthetic and perioperative management, there is ongoing uncertainty about the importance of OSA as a risk factor for post-operative complications.

Methods: A cohort study involving subjects undergoing elective surgery was conducted.

Objective: Patient reported measures of hospital care are known predictors of readmission, even after accounting for risk related to age and comorbidities. This study aimed to determine the association between patient experience of diabetes-related foot disease (DFD) hospital care and unplanned hospital readmission, with a primary focus on DFD-related readmissions and a secondary focus on all-cause readmissions.

Methods: A retrospective longitudinal cohort study was conducted by linking NSW Adult Admitted Patient Survey data with administrative hospital data for persons hospitalised with DFD identified through diagnostic and/or procedure codes.

Developing a prediction model for cognitive impairment in older adults following critical illness.

Background: New or worsening cognitive impairment or dementia is common in older adults following an episode of critical illness, and screening post-discharge is recommended for those at increased risk. There is a need for prediction models of post-ICU cognitive impairment to guide delivery of screening and support resources to those in greatest need. We sought to develop and internally validate a machine learning model for new cognitive impairment or dementia in older adults after critical illness using electronic health record (EHR) data.

Sequence-defined phosphoestamers for selective inhibition of the KRAS/RAF1 interaction.

RAS proteins are the most frequently mutated in cancer, yet they have proved extremely difficult to target in drug discovery, largely because interfering with the interaction of RAS with its downstream effectors comes up against the challenge of protein-protein interactions (PPIs). Sequence-defined synthetic oligomers could combine the precision and customisability of synthetic molecules with the size required to address entire PPI surfaces. We have adapted the phosphoramidite chemistry of oligonucleotide synthesis to produce a library of nearly one million non-nucleosidic oligophosphoester sequences (phosphoestamers) composed of units taken from synthetic supramolecular chemistry, and used a fluorescent-activated bead sorting (FABS) process to select those that inhibit the interaction between KRAS (the most prevalent, and undrugged, RAS mutant) and RAF, a downstream effector of RAS that drives cell proliferation.

How the Somatosensory System Adapts to the Motor Change in Stroke: A Hemispheric Shift?

Previous studies found that post-stroke motor impairments are associated with damage to the lesioned corticospinal tract and a maladaptive increase in indirect contralesional motor pathways. How the somatosensory system adapts to the change in the use of motor pathways and the role of adaptive sensory feedback to the abnormal movement control of the paretic arm remains largely unknown. We hypothesize that following a unilateral stroke, there is an adaptive hemispheric shift of somatosensory processing toward the contralesional sensorimotor areas to provide sensory feedback support to the contralesional indirect motor pathways.

Changes in neuronal inhibition have been implicated in age-related declines in sensorimotor performance. While indirect evidence suggests that inhibitory mechanisms are also involved in rhythm entrainment, this association has not been tested. Using magnetic resonance spectroscopy, we tested the association between dorsomedial frontal GABA+/H2O concentrations and musical rhythm production in healthy younger (n = 14; 18-35) and older (n = 12; 55-79) adults, hypothesizing that lower GABA+/H2O concentrations would be associated with increased timing error, particularly on more difficult exercises, and intra-individual variability (quantified via mean successive squared difference (MSSD)).

Introduction: Rheumatic heart disease (RHD) is underdiagnosed globally resulting in missed treatment opportunities and adverse clinical outcomes. We describe the protocol for a study which aims to co-design, implement and conduct an evaluation of a task-sharing approach to echocardiographic active case finding for early detection and management of RHD in high-risk settings in Australia and Timor-Leste.

Methods And Analysis: Echocardiograms will be obtained by trained local staff using hand-held echocardiographic devices employing the 'Single Parasternal Long Axis view with a Sweep of the Heart' (SPLASH) technique and interpreted by experts remote from the site of acquisition.
