- The study aimed to improve patient care transition from the emergency department (ED) to outpatient gastrointestinal (GI) clinics by implementing an electronic medical record (EMR) based transition-of-care (TOC) program.
- A comparative analysis of patients before and after TOC implementation showed that TOC patients were significantly more likely to schedule and attend follow-up appointments than those in the pre-TOC group.
- Notably, patients from economically disadvantaged communities benefited significantly, with those in at-risk areas being over 22 times more likely to schedule appointments post-implementation compared to their pre-TOC counterparts.
Obesity is a major health priority in the United States, as well as globally. It is associated with multiple comorbidities and reduced life expectancy. Effective management of obesity involves producing an intervention plan tailored to the individual patient.
