Models of care in secure services for people with intellectual and developmental disability: Implementing the Walkway to Wellness.

Background: Changes to policy around inpatient services for people with intellectual and developmental disability (IDD) who offend, have led to a need for services to reconsider their models of care. This has led to calls for more tailored, patient-centred care models, with less reliance solely on offence-related treatment programmes which can be unsuitable for a growing proportion of patients with more complex cognitive and behavioural difficulties. In response, the Walkway to Wellness (W2W) was developed at one National Health Service Trust providing secure services to people with IDD, with the intention of delivering a more collaborative, co-produced and goal-oriented care model that was better understood by staff and patient stakeholders.

A systematic review and narrative analysis of digital speech biomarkers in Motor Neuron Disease.

Motor Neuron Disease (MND) is a progressive and largely fatal neurodegeneritve disorder with a lifetime risk of approximately 1 in 300. At diagnosis, up to 25% of people with MND (pwMND) exhibit bulbar dysfunction. Currently, pwMND are assessed using clinical examination and diagnostic tools including the ALS Functional Rating Scale Revised (ALS-FRS(R)), a clinician-administered questionnaire with a single item on speech intelligibility.

Biocatalytic Enantioselective Synthesis of Atropisomers.

Atropisomeric compounds are found extensively as natural products, as ligands for asymmetric transition-metal catalysis, and increasingly as bioactive and pharmaceutically relevant targets. Their enantioselective synthesis is therefore an important ongoing research target. While a vast majority of known atropisomeric structures are (hetero)biaryls, which display hindered rotation around a C-C single bond, our group's long-standing interest in the control of molecular conformation has led to the identification and stereoselective preparation of a variety of other classes of "nonbiaryl" atropisomeric compounds displaying restricted rotation around C-C, C-N, C-O, and C-S single bonds.

Measuring the success of climate change adaptation and mitigation in terrestrial ecosystems.

Natural and seminatural ecosystems must be at the forefront of efforts to mitigate and adapt to climate change. In the urgency of current circumstances, ecosystem restoration represents a range of available, efficient, and effective solutions to cut net greenhouse gas emissions and adapt to climate change. Although mitigation success can be measured by monitoring changing fluxes of greenhouse gases, adaptation is more complicated to measure, and reductions in a wide range of risks for biodiversity and people must be evaluated.

Improving the Written Medical Handover.

The handover of large numbers of medical patients, during on call periods when staffing levels are reduced, is a challenge for all acute medical services. At the Royal Cornwall Hospital, a large district general hospital, we identified that foundation doctors were reviewing medical inpatients during weekend on call periods with limited written handover information. We chose to address this problem by developing an intervention, a weekend handover sticker, and piloting it.

Capacity of patients with brain metastases to make treatment decisions.

Objective: The aim of this study was to investigate medical decision-making capacity (MDC) in patients with brain metastases.

Methods: Participants were 41 adults with brain metastases with Karnofsky Performance Status scores of ≥70 who were recruited from an academic medical center and 41 demographically matched controls recruited from the community. We evaluated MDC using the Capacity to Consent to Treatment Instrument and its four clinically relevant consent standards (expressing a treatment choice, appreciation, reasoning, and understanding).

Genetic diversity of porcine group A rotavirus strains in the UK.

Rotavirus is endemic in pig farms where it causes a loss in production. This study is the first to characterise porcine rotavirus circulating in UK pigs. Samples from diarrheic pigs with rotavirus enteritis obtained between 2010 and 2012 were genotyped in order to determine the diversity of group A rotavirus (GARV) in UK pigs.

Topical calcitriol is degraded by ultraviolet light.

Calcitriol ointment has been approved for the treatment of psoriasis in many countries around the world. It may be prescribed in conjunction with phototherapy. Our purpose was to evaluate the effect of various therapeutic ultraviolet modalities on the stability of calcitriol and, conversely, to study the effects of calcitriol ointment on transmission of different forms of ultraviolet light.

An improved assay procedure and a new chemically stable ligand for cytosolic retinoic acid binding protein.

A high through-put method to assay for binding of retinoids to their cytosolic binding protein is described. The protein-bound retinoid is quantified following its complete separation from the free ligand by a single gel filtration chromatography step on Sephadex G-25 columns. The method allows a single person to process up to 100 samples per day.
