In this editorial, we provide a critical review of the article by Tang published in the , focusing on the utilization of butorphanol for epidural analgesia during labor. Our discussion encompasses recent research developments in epidural labor analgesia, specifically highlighting the current status of clinical applications of butorphanol and associated treatment approaches. Epidural analgesia is widely acknowledged as the primary method for pain management during labor, offering effective and prolonged pain relief while allowing mothers to remain alert and actively participate in the delivery process.

Article Synopsis
  • Neurocognitive Disorders (NCDs) impact cognitive abilities like memory and problem-solving, typically arising from central nervous system issues.
  • Research highlights that inflammation in the hippocampus, triggered by microglial cells, plays a key role in cognitive decline associated with NCDs.
  • The receptor TREM1, which increases during inflammation, enhances microglial activation and may be vital in the inflammatory processes linked to NCDs.

General anesthesia, as a commonly used medical intervention, has been widely applied during surgical procedures to ensure rapid loss of consciousness and pain relief for patients. However, recent research suggests that general anesthesia may be associated with the occurrence of perioperative neurocognitive disorder (PND). PND is characterized by a decline in cognitive function after surgery, including impairments in attention, memory, learning, and executive functions.

The pathophysiological regulatory mechanisms in postoperative neurocognitive disorders (PNCDs) are intricately complex. Currently, the pathogenesis of PNCDs has not been fully elucidated. The mechanism involved may include a variety of factors, such as neuroinflammation, oxidative stress, and neuroendocrine dysregulation.

This study aimed to determine the values of the half-effective dose (ED) and 95% effective dose (ED) of remimazolam besylate used in the procedural sedation of endoscopic retrograde cholangiopancreatography (ERCP). Sixty patients who fulfilled the inclusion and exclusion criteria of this study were selected. Sufentanil was administered intravenously and remimazolam besylate was administered 2 min later.

Article Synopsis
  • The study aimed to evaluate the effects of subanesthetic-dose S-ketamine on reducing postoperative opioid use while assessing its safety and effectiveness in managing pain.
  • Four hundred twenty patients were randomly assigned to three groups: a control group, a group receiving 0.2 mg/kg S-ketamine, and another receiving 0.3 mg/kg S-ketamine, with various pain and safety metrics tracked post-surgery.
  • Results showed that the 0.3 mg/kg group experienced significantly lower pain scores and reduced opioid usage compared to the control group, indicating that S-ketamine can effectively help manage postoperative pain without increasing adverse effects.

Article Synopsis
  • The study investigates whether smoking is an independent risk factor for postoperative cognitive impairment in patients undergoing general anesthesia.
  • It involved 112 patients undergoing painless gastroscopy where baseline cognitive scores were measured, and changes were observed immediately after anesthesia and three days post-op.
  • Results showed significant cognitive differences immediately after anesthesia between smokers and non-smokers, with smoking linked to increased postoperative cough and potentially contributing to cognitive dysfunction in early postoperative recovery.
