Inaccuracies in angiographic staging of renal cell carcinoma.

In 54 patients with renal cell carcinoma, the angiographic T category of the International Union Against Cancer (UICC) clasification correlated with the histopathologic (P) staging in only 44.4 per cent. Staging of the primary lesion in renal cell carcinoma must be based on the P category and not the angiographic appearances.

Proximal and distal reconstruction of the extensor mechanism for patellar subluxation.

Proximal and distal reconstruction of the leg extensor mechanisms has been developed and performed in 346 cases over a period of 25 years. Proximal extensor mechanism reconstruction produces very satisfactory results in over 70% of cases and continues to be the basic principle of surgical treatment for patellar subluxation. In patients with Q angles (angle between line of the quadriceps force and the direction of the patellar tendon) greater than 10 degrees distal reconstruction (patellar tendon transfer) is done in addition to, not in lieu of, the dynamic proximal reconstruction.

Memory for constrained and preselected movement location and distance.

These experiments assessed the interrelationship between location and distance cues in the coding of movements. In separate experiments subjects recalled either the terminal location or the distance of constrained (Experiment 1) or preselected (Experiment 2) movements following a 15-sec retention interval. Changes in direction amd amplitude of starting position were used to ascertain whether recall errors were related to these changes.

Prevention of ankle sprains.

Ankles are sprained when supported on an unstable foundation, while too rididly fixed to the playing surface, or when forced into unnatural positions by extrinsic muscle tightness. The unstable foundation may be the shoe itself, a chuck-hole, or another player's foot. Undue fixation may be by 1-inch mud cleats, baseball spikes, or a modern wrestling mat.

Assessment of diagnostic value of technetium-99m pyrophosphate myocardial scintigraphy in 80 patients with possible acute myocardial infarction.

The diagnostic value of technetium-99m-pyrophosphate (Tc-pyrophosphate) myocardial scintigraphy was determined in 80 consecutive patients who had been admitted to the coronary care unit in order to rule out an acute myocardial infarction. Scintigraphic findings obtained within 5 days of admission were correlated with the final cardiac diagnosis determined for each patient. Significant myocardial uptake of Tc-pyrophosphate (positive scans) occurred in 13 of 22 patients (59%) who had enzyme and/or electrocardiographic proven acute myocardial infarct: 3 out of 5 with transmural myocardial infarct, 9 of 16 with nontransmural myocardial infarct, and 1 patient with left bundle-branch block.

Nitrogen-13-labeled ammonia for myocardial imaging.

Cyclotron-produced nitrogen-13 (half-life 10 min), as labeled ammonia (13NH4+), has been evaluated as a myocardial perfusion imaging agent. The regional myocardial uptake of 13NH4+ has been shown to be proportional to regional tissue perfusion in animal studies. Intravenously administered 13NH4+ is rapidly cleared from the circulation, being extracted by the liver (15%), lungs, myocardium (2%-4%), brain, kidney, and bladder.

Noninvasive evaluation of regional myocardial perfusion in 112 patients using a mobile scintillation camera and intravenous nitrogen-13 labeled ammonia.

The short half-life positron emitter 13N, as labeled ammonia (13NH4+), was evaluated as a myocardial imaging agent. Regional myocardial uptake of 13NH4 correlated with the distribution of labeled microspheres in experimental myocardial infarction. Using intravenous 13NH4+, myocardial scintigraphy was performed in 85 cardiac patients and 27 normal subjects.

Comparison of single and biplane ejection fractions in patients with Ischaemic heart disease.

Simultaneous biplane left ventriculography was performed in 59 patients with ischaemic heart disease. A comparison was made of the frontal (EFF), lateral (EFL), and biplane (EFB) ejection fractions. Discrepancies between the three measurements in the same patient were frequent observed.

Coronary arteriographic study of mild angina.

The results of coronary arteriography, exercise, and pacing stress testing in 50 young patients with mild angina are analysed. There was a significant correlation between the results of stress tests and the secerity of disease. There were, however, patients with severe disease and negative tests.

Mechanisms of immunity in typhus infections. IV. Failure of chicken embryo cells in culture to restrict growth of antibody-sensitized Rickettsia prowazeki.

Rickettsia prowazeki, pretreated with typhus immune human serum, readily infects, and grows in, chicken embryo cells in culture. This finding is similar to those of previous studies which showed that typhus rickettsiae, pretreated with immune serum, grow in cells of the yolk sac of embryonated hen eggs and in the cells of the midgut of the human body louse. In contrast, identically treated typhus rickettsiae were destroyed by human macrophages in culture.
