Psychological interventions for patients with delirium in intensive care: A scoping review protocol.

The objective of this scoping review is to investigate psychological interventions developed, evaluated, and considered for patients with delirium in intensive care units (ICU). Data will be extracted from sources of evidence that address interventions for delirium-related distress and/or cognitive impairments in the adult ICU population, suitable for delivery by or under the supervision of a psychological practitioner. ICU delirium is a common and impactful condition that adversely affects patient outcomes, including prolonged hospitalisation and deteriorating mental health.

Prolonged disorder of consciousness: giving a second opinion on the best interests of a person.

Patients with a prolonged disorder of consciousness have a neurological disorder. Few neurologists undertake assessment and management after the acute phase of the person's illness, even though some of their patients with progressive neurological disorders become persistently unconscious. Their clinical evaluation and deciding on their best interests require full use of a neurologist's expertise, which is intellectually, emotionally and clinically challenging.

Sensorimotor functioning changes in response to global exercise versus handwriting upper limb exercise training in Parkinson's disease, results from a phase II randomised controlled trial.

Introduction: People with Parkinson's disease (PwPD) present motor alterations which can impact daily life tasks that require speed and/or accuracy of movement.

Objective: A sub analysis of NCT01439022, aiming to estimate the extent to which two different exercise training protocols (global and handwriting upper limb exercise training) impact reaction time, travel speed, and accuracy in PwPD.

Methods: Seventy PwPD, right-side dominant were randomised 1:1 into two six-month training protocol groups; 35 PwPD performed global exercise training and 35 performed specific training (handwriting upper limb exercise movements).

Does a service provide safe, effective rehabilitation? An evaluation method for providers and purchasers.

Background: Independent organisations monitor the safety and governance of clinical services but do not assess specialist expertise. Peer review can assess service capability but is resource-intense and infeasible.

The Problem: How can you ensure a service provides safe, effective rehabilitation? You ask them to provide data as evidence that they can be trusted to do so.

Knee osteoarthritis pendulum therapy: evaluation and a randomised, single-blind feasibility clinical trial.

Background: Exercise is recommended as the first-line management for knee osteoarthritis (KOA); however, it is difficult to determine which specific exercises are more effective. This study aimed to explore the potential mechanism and effectiveness of a leg-swinging exercise practiced in China, called 'KOA pendulum therapy' (KOAPT). Intraarticular hydrostatic and dynamic pressure (IHDP) are suggested to partially explain the signs and symptoms of KOA.

Introduction: Calcium is required for coagulation, cardiac output, and peripheral vascular resistance. Between 85% and 94% of trauma patients treated with massive blood transfusion develop hypocalcemia. A general theory of rehabilitation: Rehabilitation catalyses and assists adaptation to illness.

Background: There is no general theory of rehabilitation, only definitions and descriptions, with the biopsychosocial model of illness as a structure.

Objective: To develop a general theory of rehabilitation that explains how healthcare rehabilitation changes outcomes and to evaluate its validity.

Need: A general rehabilitation theory would help research, improve services, increase understanding, modify resource allocation and explain some anomalies, such as how rehabilitation helps when no natural recovery occurs.

Anomalies in the review process and interpretation of the evidence in the NICE guideline for chronic fatigue syndrome and myalgic encephalomyelitis.

Chronic fatigue syndrome/myalgic encephalomyelitis (CFS/ME) is a disabling long-term condition of unknown cause. The National Institute for Health and Care Excellence (NICE) published a guideline in 2021 that highlighted the seriousness of the condition, but also recommended that graded exercise therapy (GET) should not be used and cognitive-behavioural therapy should only be used to manage symptoms and reduce distress, not to aid recovery. This U-turn in recommendations from the previous 2007 guideline is controversial.

Before choosing a uniform name for the PMR specialty, we must define the specialty.

In this commentary, I develop the recent proposal to choose a uniform name globally for our specialty ("A call for a single international name for the specialty"), suggesting it is premature and we must first agree on the core, central features characterizing a specialist. The question is, what is our specialty? The scope and content vary significantly between and within countries. If we can agree on the nature and scope of the specialty, a one-word name may arise that people and countries could choose to use.

Rehabilitation potential: A critical review of its meaning and validity.

Background: The concept of rehabilitation potential emerged in 1950 as a way to select people for rehabilitation; it is also used to limit access to services.

Objective: To elucidate the meaning(s) of rehabilitation potential and whether it is valid in selecting patients for rehabilitation, whether as an inpatient, outpatient, or in the community.

Method: A non-systematic review of how it has been used, a structured discussion of its potential meanings, an exploration of the evidence in support of selecting people who might benefit from rehabilitation, and a discussion of the concept of rehabilitation potential.

High pathogenicity AI (HPAI) has emerged in consecutive seasons within the UK to cause the largest outbreaks recorded. Statutory measures to control outbreaks of AI virus (AIV) at poultry farms involve disposal of all birds on infected premises.

Slow development of woodland vegetation and bird communities during 33 years of passive rewilding in open farmland.

Passive rewilding is a potential tool for expanding woodland cover and restoring biodiversity by abandoning land management and allowing natural vegetation succession to occur. Land can be abandoned to passive rewilding deliberately or due to socio-economic change. Despite abandonment being a major driver of land use change, few have studied the long-term outcomes for vegetation and biodiversity in Western Europe.

Patient and Caregiver-Derived Health Service Improvements for Better Critical Care Recovery.

Objectives: To engage critical care end-users (survivors and caregivers) to describe their emotions and experiences across their recovery trajectory, and elicit their ideas and solutions for health service improvements to improve the ICU recovery experience.

Design: End-user engagement as part of a qualitative design using the Framework Analysis method.

Setting: The Society of Critical Care Medicine's THRIVE international collaborative sites (follow-up clinics and peer support groups).

What is rehabilitation? From 1994 to 2021, while I was privileged to be Editor of Clinical Rehabilitation, I explored this in editorials. I also encouraged and selected submissions that considered, in one way or another, the central features of rehabilitation. Why? Because when I started in rehabilitation, the general attitude among doctors and other healthcare professionals was that rehabilitation was pleasant but with no evidence of effectiveness.

Building an integrated serosurveillance platform to inform public health interventions: Insights from an experts' meeting on serum biomarkers.

The use of biomarkers to measure immune responses in serum is crucial for understanding population-level exposure and susceptibility to human pathogens. Advances in sample collection, multiplex testing, and computational modeling are transforming serosurveillance into a powerful tool for public health program design and response to infectious threats. In July 2018, 70 scientists from 16 countries met to perform a landscape analysis of approaches that support an integrated serosurveillance platform, including the consideration of issues for successful implementation.

Modification of social determinants of health by critical illness and consequences of that modification for recovery: an international qualitative study.

Objectives: Social determinants of health (SDoH) contribute to health outcomes. We identified SDoH that were modified by critical illness, and the effect of such modifications on recovery from critical illness.

Design: In-depth semistructured interviews following hospital discharge.

Operative management of non-elective incisional hernia reduces readmission in a national database.

Purpose: The aim of this project was to compare patient characteristics, overall efficacy, and readmission events following operative vs non-operative management modalities of non-elective patients presenting with symptomatic incisional hernias.

Methods: This study is a retrospective study of patients and patient demographics that presented as non-elective hospitalizations with symptomatic incisional hernia. Analysis of patients and characteristics utilized the National Readmission Database from 2010 to Q3 of 2015, delineating patient factors and outcomes following operative or non-operative management of hernias.

Introduction: Stillbirth is a global health problem having many emotional, social and economic consequences. India has the largest number of stillbirths per year in the world.

Objective: The objective of this study is to review the causes of stillbirth and classify the causes into maternal, foetal and placental causes and further classify causes by relevant condition at death (ReCoDe) classification.

Management of traumatic brain injury: practical development of a recent proposal.

A recent article identified weaknesses in the management of patients with traumatic brain injury (TBI). The authors suggested some reasons but overlooked two of the reasons for the low quality of services: a lack of resources and a systemic failure to organise rehabilitation services. They suggested early involvement of a condition-specific service with a new 'neuroscience clinician' and additional neuro-navigators, but the evidence shows this approach does not work.
