The relative conservation of the influenza hemagglutinin (HA) stem compared to that of the immunodominant HA head makes the HA stem an attractive target for broadly protective influenza vaccines. Here we report the first-in-human, dose-escalation, open-label trial (NCT04579250) evaluating an unadjuvanted group 2 stabilized stem ferritin nanoparticle vaccine based on the H10 A/Jiangxi-Donghu/346/2013 influenza HA, H10ssF, in healthy adults. Participants received a single 20 mcg dose (n = 3) or two 60 mcg doses 16 weeks apart (n = 22).

Background: Acute respiratory distress syndrome (ARDS), a life-threatening condition during critical illness, is a common complication of COVID-19. It can originate from various disease etiologies, including severe infections, major injury, or inhalation of irritants. ARDS poses substantial clinical challenges due to a lack of etiology-specific therapies, multisystem involvement, and heterogeneous, poor patient outcomes.

Importance: Communication and adoption of modern study design and analytical techniques is of high importance for the improvement of clinical research from observational data.

Objective: To compare a modern method for statistical inference, including a target trial emulation framework and doubly robust estimation, with approaches common in the clinical literature, such as Cox proportional hazards models.

Design, Setting, And Participants: This retrospective cohort study used longitudinal electronic health record data for outcomes at 28-days from time of hospitalization within a multicenter New York, New York, hospital system.

Background: Acute respiratory distress syndrome (ARDS), a life-threatening condition characterized by hypoxemia and poor lung compliance, is associated with high mortality. ARDS induced by COVID-19 has similar clinical presentations and pathological manifestations as non-COVID-19 ARDS. However, COVID-19 ARDS is associated with a more protracted inflammatory respiratory failure compared to traditional ARDS.

Influenza infection induces lung epithelial cell injury via programmed cell death. Glutathione, a potent antioxidant, has been reported to be associated with influenza infection. We hypothesized that lung epithelial cell death during influenza infection is regulated by glutathione metabolism.

Importance: Communication and adoption of modern study design and analytical techniques is of high importance for the improvement of clinical research from observational data.

Objective: To compare (1) a modern method for causal inference including a target trial emulation framework and doubly robust estimation to (2) approaches common in the clinical literature such as Cox proportional hazards models. To do this, we estimate the effect of corticosteroids on mortality for moderate-to-severe coronavirus disease 2019 (COVID-19) patients.

Multiple studies have investigated the role of blood circulating proteins in COVID-19 disease using the Olink affinity proteomics platform. However, study inclusion criteria and sample collection conditions varied between studies, leading to sometimes incongruent associations. To identify the most robust protein markers of the disease and the underlying pathways that are relevant under all conditions, it is essential to identify proteins that replicate most widely.

Article Synopsis
  • Idiopathic pulmonary fibrosis (IPF) is a serious and progressive lung disease with unknown causes and mechanisms.
  • Research shows that levels of circulating cell-free double-stranded DNA (ccf-dsDNA) are higher in patients with fast-progressing IPF than in slow progressors and healthy individuals.
  • ccf-dsDNA levels are linked to key metabolic processes in patients with IPF, specifically amino acid, energy, and lipid metabolism.

To develop evidence-based recommendations on best practices for delivery of clinical preventive services by chiropractors and to offer practical resources to empower provider applications in practice. Clinical practice guideline based on evidence-based recommendations of a panel of practitioners and experts on clinical preventive services. Synthesizing the results of a literature search for relevant clinical practice guidelines and systematic reviews, a multidisciplinary steering committee with training and experience in health promotion, clinical prevention, and/or evidence-based chiropractic practice drafted a set of recommendations.

Objective: The purpose of this article is to explore concerns regarding sections of the federal workers' compensation law that apply to the treatment and management of work-related injuries of federal employees by chiropractors, and to offer a call to action for change.

Discussion: A 1974 amendment to the Federal Employees' Compensation Act (FECA) stipulates that chiropractic services rendered to injured federal workers are reimbursable. However, the only reimbursable chiropractic treatment is "manual manipulation of the spine to correct a subluxation as demonstrated by X-ray to exist.

Article Synopsis
  • A clinical practice guideline (CPG) was developed to establish best practices for chiropractic management of chronic musculoskeletal (MSK) pain, based on evidence from expert consensus.
  • A diverse panel of 62 experts utilized a formal Delphi consensus process to evaluate and agree on recommendations for managing five prevalent chronic MSK pain conditions: low-back pain, neck pain, tension headaches, osteoarthritis, and fibromyalgia.
  • The CPG emphasizes nonpharmacological treatments, promotes collaboration among providers, and aims to enhance patient outcomes through consistent and informed chiropractic care.

An attenuated Plasmodium falciparum (Pf) sporozoite (SPZ) vaccine, PfSPZ Vaccine, is highly protective against controlled human malaria infection (CHMI) 3 weeks after immunization, but the durability of protection is unknown. We assessed how vaccine dosage, regimen, and route of administration affected durable protection in malaria-naive adults. After four intravenous immunizations with 2.

Indoor navigation technology is needed to support seamless mobility for the visually impaired. This paper describes the construction and evaluation of an inertial dead reckoning navigation system that provides real-time auditory guidance along mapped routes. Inertial dead reckoning is a navigation technique coupling step counting together with heading estimation to compute changes in position at each step.

Objective: Although a number of guidelines addressing manipulation, an important component of chiropractic professional care, exist, none to date have incorporated a broad-based consensus of chiropractic research and clinical experts representing mainstream chiropractic practice into a practical document designed to provide standardized parameters of care. The purpose of this project was to develop such a document.

Methods: Development of the document began with seed materials, from which seed statements were distilled.

Article Synopsis
  • A case study highlights a 40-year-old woman with a 2-level cervical disk herniation and radicular symptoms treated conservatively through chiropractic care.
  • The patient experienced neck pain and finger paresthesia, but diagnostic tests showed normal nerve function, while MRI revealed significant disk protrusion causing spinal canal narrowing.
  • After receiving high-velocity, low-amplitude chiropractic manipulations along with other conservative treatments, her symptoms fully resolved within 90 days, and she remained symptom-free for a year without any adverse effects.
