Purpose: The absence of evidence based general guidelines for radiographic follow-ups of pediatric diaphyseal forearm fractures treated with ESIN results in an arbitrary array of X-ray examinations. In most pediatric traumatology departments, an X-ray check is carried out 4 weeks after ESIN osteosynthesis of forearm shaft fractures to detect incipient consolidation of the fracture. However, the elevated sensitivity to ionizing radiation requires special precautions in the pediatric populations.

Haematological malignancies affect 12·5 in 100 000 pregnancies. Over the past two decades, the number of haematological malignancies in pregnancy has substantially increased. Life-threatening haematological malignancies in pregnancy, such as acute leukaemia and aggressive lymphomas, pose a unique therapeutic challenge: clinicians must consider both maternal and fetal wellbeing, aiming to deliver optimal curative therapy for the patient and a successful pregnancy outcome.

Large vessel vasculitides (LVV) such as giant cell arteritis, Takayasu arteritis and aortitis/periaortitis are characterised by immune-mediated inflammation of medium to large arteries. Clinical disease manifestations can be non-specific and diagnostic imaging plays an important role in the diagnostic pathway. In recent years, FDG PET/CT has proven to be a powerful metabolic tool that can provide a wholed body, non-invasive assessment of vascular inflammation.

A 12-year-old girl with no previous comorbidities presented with a left gluteal swelling. 18 F-FDG PET/CT demonstrated heterogenous, moderate hypermetabolism localizing to a very large pelvic soft tissue mass with extension through the left greater sciatic notch into the left deep gluteal space. Core biopsy of the mass revealed INI1-negative myoepithelial carcinoma.

Article Synopsis
  • A 6-month-old boy had soft tissue swelling and CT scans showed multiple bone lesions indicating a serious condition.
  • PET/CT scans revealed hypermetabolic lesions in several areas, and a biopsy of one lesion confirmed it was Langerhans cell histiocytosis.
  • After chemotherapy, further testing showed another lesion that was diagnosed as juvenile xanthogranuloma, highlighting a rare case of mixed histiocytosis.

  • Bucket-handle meniscal tears are primarily treated with arthroscopic surgery, but the timing of the surgery has unclear effects on recovery outcomes.
  • A study involving 60 patients showed that an average wait of 14.4 days for surgery did not significantly affect reoperation rates, pain levels, or return to sports.
  • The findings suggest that surgical timing is not a predictor of post-surgery success, indicating that careful planning and expertise matter more than urgency in treatment.

99m Tc-MAG3 diuresis renography is a noninvasive functional imaging technique used to assess clearance of radiotracer in a dilated urinary tract under high diuresis. It is commonly performed in patients to diagnose functionally significant urinary tract obstruction. In some pediatric patients, sedation is required to enable imaging.

Article Synopsis
  • Proximal humeral fractures in children are uncommon and typically managed non-operatively, especially in those under ten, with treatment decisions influenced by age and the degree of fracture angulation.
  • A study assessed the agreement among pediatric trauma surgery experts in Germany regarding the assessment of fracture angulation and treatment decisions, revealing moderate agreement among them on fracture angulation measurements and fair agreement on treatment methods.
  • The findings highlight a need for a standardized approach to measuring fracture angulation in children to improve treatment consistency, suggesting that adopting a commonly accepted method may enhance decision-making over relying on complex techniques.

Post-traumatic joint stiffness (PTJS) is accompanied by a multidimensional disturbance of joint architecture. Pharmacological approaches represent promising alternatives as the traumatic nature of current therapeutic standards may lead to PTJS' progression. Losartan is an auspicious candidate, as it has demonstrated an antifibrotic effect in other organs.

Purpose: Personality traits, such as dispositional optimism and pessimism, have impact on a variety of health-related problems. Influence on outcome in total knee arthroplasty (TKA) could only be shown for other personality trait concepts, but not for dispositional optimism/pessimism. This study aims to examine the association of dispositional optimism/pessimism with pre-operative joint function and post-operative outcome in TKA.

Background: The efficacy and safety of enhanced recovery after surgery (ERAS) protocols for patients undergoing total knee arthroplasty (TKA) have been generally proven. Previous studies investigating patients undergoing simultaneous bilateral TKA (SBTKA) focused on complications, mortality, and pain and did not examine patients' functional limitations. Therefore, the aim of this study was to investigate to what extent patients undergoing SBTKA are able to meet functional discharge criteria originally designed for their counterparts undergoing unilateral TKA (UTKA) in an ERAS setting.

Background: Group medical visits (GMV) have been shown to improve metrics in patients with type 2 diabetes mellitus (DM). Overlook Family Medicine, a teaching residency program, anticipated that medical residents trained in the GMV model of care by interdisciplinary team members may improve cholesterol, HbA1C, BMI, and blood pressure in patients. The objective of this study was to compare metrics between group 1: GMV patients with DM whose primary care provider (PCP) was an attending physician/nurse practitioner (NP) and group 2: GMV patients with DM whose PCP was a family medicine (FM) medical resident receiving GMV training.

Article Synopsis
  • The study examined the effects of atorvastatin on posttraumatic joint contracture using a rat model, involving 48 rats divided into atorvastatin and control groups.
  • Despite atorvastatin reducing the number of myofibroblasts and collagen in the joint capsule, it did not significantly improve joint mobility or prevent contracture formation.
  • The findings indicate that the primary cause of joint contracture was arthrogenic, and atorvastatin's antifibrotic properties did not translate into better joint function in this context.

Introduction: Delirium is associated with new onset dementia and accelerated cognitive decline; however, its pathophysiology remains unknown. Cerebral glucose metabolism previously seen in delirium may have been attributable to acute illness and/or dementia. We aimed to statistically map cerebral glucose metabolism attributable to delirium.

The prognosis of recurrent malignant peripheral nerve sheath tumors (MPNST) is dismal, with surgical resection being the only definitive salvage therapy. Treatment with chemoradiation approaches has not significantly improved patient outcomes. Similarly, trials of therapies targeting MPNST genomic drivers have thus far been unsuccessful.

Article Synopsis
  • - Infections with certain bacteria have risen significantly in recent years, highlighting the need for effective prevention strategies as conventional antibiotics are becoming less effective and often lead to recurring infections.
  • - Current advanced vaccines focus on preventing symptoms after infection starts but don’t stop the bacteria from colonizing the gut, allowing for asymptomatic cases to persist.
  • - New research on glycoconjugate vaccines using synthetic oligosaccharides shows promise; these vaccines prevented infections in mice and improved gut health more effectively than toxin-targeting vaccines by inducing specific antibodies and maintaining healthy gut microbiota.
