Perianal endometriosis is an infrequent form of extragenital endometriosis. A case of endometriosis in an episiotomy scar with involvement of the external anal sphincter is presented. The question of treatment is discussed.

They were treated with nonsteroidal anti-inflammatory drugs (NSAIDs) and oral contraceptive pills (OCPs) and underwent a laparoscopy to determine the etiology of pelvic pain.
