The development and implementation of a system to improve voluntary reporting of adverse drug reactions (ADRs) is described. A preliminary study comparing three concurrent voluntary reporting systems at a 327-bed university teaching hospital demonstrated the ability of medical records personnel to identify ADRs that were documented in the medical record but not reported under the voluntary system. It was concluded that a concurrent voluntary reporting system would be coupled with a medical record review process that would focus reporting on clinically important and unexpected ADRs.

A novel instructional strategy for clinical education, the "Pharmacist Assisted Self-Medication Case Report," was introduced into a community practice externship in 1982. The purpose was to create an experimental tool which developed informational and problem solving skills of pharmacy students in the area of OTC self medication. The case report requires that the student conduct a patient interview, evaluate the patient's condition, decide on an appropriate recommendation and provide appropriate consultation.

A description of a ten week, full-time, elective sterile products clerkship designed for fifth year pharmacy students is presented. The course refines the student's written and oral communication skills and meets the NCCLVP standard for education of IV therapy personnel.
