Based on their experience with the operative treatment of 20 patients with acute thrombosis of major arteries of extremities the authors recommend if indicated to combine reconstructive operation on the arteries with the artificial arterio-venous anastomosis. The authors have developed a classification of such associated operations which facilitates choice of their variants dependent on the state of the distal vascular bed.

The article describes the method of quantitative intraoperative assessment of patency of the distal vascular bed and the transplant based on the imitation of blood flow in the vascular bed located lower to the occlusion level. It can be fulfilled by introduction of the liquid under permanent pressure of 120 mm Hg and measurement of its volume per 1 min. The method was used in 85 patients.

The prostanoid status was assessed in 138 patients with coronary atherosclerosis and clinical signs of stable angina. Plasma prostaglandin F2 alpha and E1, and stable prostacyclin and thromboxane metabolites were measured radioimmunologically, using standard kits. A relationship is demonstrated between the severity of clinical manifestations, and pattern and magnitude of change in individual prostanoids.

Results of surgical treatment of 203 embolisms in arteries of the extremities, including 127 embolisms in intact arteries, and 76 embolisms in atherosclerotic ones, are reported. Post-embolectomy regression of mild ischemia is shown to be unrelated to the condition of the artery. Severe ischemia in atherosclerotic arteries does not benefit from embolectomy alone; it must be supplemented by reconstructive surgery.

The tactics of treatment and diagnosis used in 130 patients with embolism of the pulmonary artery (EPA) are discussed. All of them were examined by angiopulmonography including the method of selective contrast. Angiographic symptoms of EPA were found in 90 patients.

The authors propose a scheme for the estimation of severity of ischemic alterations in the extremity in acute ischemia in order to choose the optimal methods of treatment. In cases of mild ischemia the clinical estimation is sufficient, with the degree of the acute ischemia syndrome taken into account. In cases of severe ischemia the clinical assessment is found to be insufficient.
