We describe an unusual case of a coronary perforation presenting as a large pseudoaneurysm. An elderly woman who had undergone percutaneous coronary intervention several months earlier presented with shortness of breath. Cardiac catheterization revealed a large pseudoaneurysm arising from the proximal circumflex artery.
