Vitreous substitutes and tamponades occupy the vitreous cavity following vitrectomy in the management of various conditions such as retinal detachment, macular hole, and diabetic retinopathy. Such replacements can be for the short term (gases such as sulfur hexafluoride (SF6) and perfluoropropane (C3F8) or long term (such as silicone oils). Certain substitutes such as perfluorocarbon liquids are used only transiently during surgery as "a third hand" or rarely till a few days post surgery.

Article Synopsis
  • The study investigates the learning curves of two vitreoretinal surgeons (one self-trained and one supervised) performing posterior segment diagnostic endoscopy over a span of six years, from 2017 to 2023.
  • Historical data was reviewed, analyzing the time taken for each procedure to quantify how quickly the surgeons improved their skills based on the number of cases performed.
  • Results indicated that the self-trained surgeon required approximately 20 cases for skill stabilization, while the supervised surgeon reached stability after about 10 cases, suggesting important insights for developing training protocols in endoscopic techniques.

Article Synopsis
  • The study aims to evaluate antibiotic susceptibility in endophthalmitis cases from the Endophthalmitis Management Study (EMS) and compare it with the current practices of Indian vitreoretinal specialists.
  • Methods included microbiological analyses like microscopy, culture-susceptibility testing, PCR, and NGS, with significant results defined as p < 0.05.
  • Results indicated that gram-positive cocci (GPC) were mostly susceptible to vancomycin and gram-negative bacilli (GNB) to colistin; VRSI members primarily preferred using a combination of vancomycin and ceftazidime for treatment.

Article Synopsis
  • The study aimed to determine if an inflammation score (IS) from eye structures could indicate the severity of endophthalmitis in patients after cataract surgery.
  • In a controlled trial, 175 adults diagnosed with endophthalmitis received different treatments based on their inflammation scores and underwent comprehensive microbiological testing, revealing key correlations between high IS and worse patient outcomes.
  • Findings showed that higher IS was linked to more severe endophthalmitis, including faster symptom onset and a higher likelihood of Gram-negative infections, while two-thirds of patients improved vision post-treatment.

Infectious endophthalmitis is a severe intraocular infection caused by bacteria, or less commonly by fungi. It can occur after penetrating eye procedures, trauma, or the spread of infection from contiguous structures or via emboli from distant organs. Because of the time-critical nature of the treatment, endophthalmitis is treated with the clinical diagnosis and modified by the microbiological report of the intraocular contents.
