Background: The real incidence of atrial arrhythmia (AA) after patent foramen ovale (PFO) closure and whether this complication can be prevented remain unknown. We assessed whether flecainide is effective to prevent AA during the first 3 months after PFO closure, and whether 6 months of treatment with flecainide is more effective than 3 months to prevent AA after PFO closure.

Methods: AFLOAT (Assessment of Flecainide to Lower the Patent Foramen Ovale Closure Risk of Atrial Fibrillation or Tachycardia Trial) is a prospective, multicentre, randomized, open-label, superiority trial with a blind evaluation of all the end points (PROBE [Prospective Randomized Open, Blinded End Point] design).

Article Synopsis
  • Cardiogenic shock (CS) is a major cause of in-hospital deaths after acute coronary syndromes (ACS), with nearly 50% mortality, highlighting the need for personalized risk prediction.
  • The ORBI score, designed to predict in-hospital CS in ACS patients undergoing percutaneous coronary intervention (PCI), has shown varying effectiveness between genders, necessitating improved risk assessment methods.
  • A new score called SEX-SHOCK was developed, incorporating key health indicators, and demonstrated better predictive ability for both sexes compared to the ORBI score, thus advancing risk prediction strategies in ACS management.

Background: The best management of symptomatic patients with low-gradient (LG) severe aortic stenosis (AS) and preserved left ventricular ejection fraction (LVEF) has not been established. The Randomised study for the Optimal Treatment of symptomatic patients with low-gradient severe Aortic valve Stenosis (ROTAS) trial aimed to assess the superiority of aortic valve replacement (AVR) versus medical treatment (MT) in this specific group of AS patients.

Methods: Patients with symptomatic LG severe AS and preserved LVEF (>50%) underwent dobutamine stress echocardiography and/or CT-aortic calcium score to confirm AS severity and were then randomised 1:1 to AVR or MT.

Aims: Assessing right heart function is challenging, particularly when significant tricuspid regurgitation (TR) is present. Amongst available echocardiographic techniques for assessment, literatures suggest that strain imaging may be more reliable and less susceptible to loading conditions. Thus, we aimed to assess the validity of right atrial (RA) and right ventricular (RV) strain relative to conventional metrics as well as their utility in predicting patient outcomes in TR.

Article Synopsis
  • A 3-month-old female English setter in Quebec presented with severe respiratory distress, leading to the discovery of fungal spores in its lungs during bronchoalveolar lavage.
  • The dog’s health rapidly worsened within 24 hours and it was euthanized; post-mortem tests confirmed the presence of lycoperdonosis, a fungal infection from inhaling mushroom spores.
  • This is the first documented case of lycoperdonosis in eastern Canada, highlighting the need for veterinarians to consider spore inhalation in similar acute respiratory cases and to take detailed patient histories.

Background: The influence of permanent pacemaker implantation upon outcomes after transcatheter aortic valve implantation (TAVI) remains controversial.

Aims: To evaluate the impact of permanent pacemaker implantation after TAVI on short- and long-term mortality, and on the risk of hospitalization for heart failure.

Methods: Data from the large FRANCE-TAVI registry, linked to the French national health single-payer claims database, were analysed to compare 30-day and long-term mortality rates and hospitalization for heart failure rates among patients with versus without permanent pacemaker implantation after TAVI.

Aims: Aortic stenosis (AS) is causing myocardial damage and replacement is mainly indicated based on symptoms. Non-invasive estimation of myocardial work (MW) provides a less afterload-dependent too for assessing myocardial function. We sought to look at the impact of transcatheter aortic valve implantation (TAVI) on the myocardium at long-term follow-up and according to current indications.

Published studies on the validity of using quantitative MRI measures of pre- and postoperative spinal cord (SC) compression as prognostic indicators for dogs undergoing surgery for intervertebral disc extrusion (IVDE) are currently limited. The aim of this retrospective analytical study was to describe the volume of postoperative residual extradural material (VREM) and the ratio of the cross-sectional area (CSA) of maximum SC compression to the CSA of SC in a compression-free intervertebral space as MRI measures of preoperative and postoperative compression (residual spinal cord compression, RSCC), and to compare these measures between the neurological outcome in a group of dogs. Inclusion criteria were dogs that underwent surgery for thoracolumbar IVDE, were imaged pre- and immediately postoperatively by MRI, and had a neurological follow-up examination 2 to 5 weeks postoperatively.

New-onset conduction disturbances, including left bundle branch block and permanent pacemaker implantation, remain a major issue after transcatheter aortic valve implantation. Preprocedural risk assessment in current practice is most often limited to evaluation of the baseline electrocardiogram, whereas it may benefit from a multimodal approach, including ambulatory electrocardiogram monitoring and multidetector computed tomography. Physicians may encounter equivocal situations during the hospital phase, and the management of follow-up is not fully defined, despite the publication of several expert consensuses and the inclusion of recommendations regarding the role of electrophysiology studies and postprocedural monitoring in recent guidelines.

Article Synopsis
  • Guidelines suggest aortic valve replacement for severe aortic stenosis when patients exhibit symptoms or reduced left ventricular ejection fraction, which indicates advanced disease severity.
  • This study aimed to analyze changes in myocardial work for patients undergoing transcatheter aortic valve implantation (TAVI) by evaluating echocardiographic data before and after the procedure.
  • The results showed a significant decrease in mean transaortic pressure gradient and improvements in myocardial work indices post-TAVI, indicating that these measures could help better understand the left ventricular effects of aortic stenosis and treatment outcomes.
