Objective: To develop an improved score for prediction of severe infection in patients with systemic lupus erythematosus (SLE), namely, the SLE Severe Infection Score-Revised (SLESIS-R) and to validate it in a large multicentre lupus cohort.

Methods: We used data from the prospective phase of RELESSER (RELESSER-PROS), the SLE register of the Spanish Society of Rheumatology. A multivariable logistic model was constructed taking into account the variables already forming the SLESIS score, plus all other potential predictors identified in a literature review.

Article Synopsis
  • The study analyzed how common major central nervous system issues are in patients with systemic lupus erythematosus (SLE) and their impact on survival and mortality.
  • Out of 3,591 SLE patients, 11.5% experienced significant CNS neuropsychiatric problems, leading to higher annual mortality rates, particularly from cerebrovascular disease and organic brain syndrome.
  • The findings indicate that major CNS complications significantly increase mortality risk, with specific issues like CVD and organic brain syndrome having the worst survival outcomes.

In the last two decades, ultrasound has been fully implemented in the diagnosis and management of rheumatoid arthritis. Several studies have been published that have demonstrated better availability of this technique in the identification of elementary inflammatory joint and tendon injuries, as joint and tendon sheath synovial hypertrophy, joint effusion, and Power Doppler signal. Ultrasonography has good properties to identify changes with different treatments, have predictive value for relapse in patients in clinical remission and in structural damage.

Objectives: Identifying early predictors of response to biological agents is important for both the individual patient and health economics. The aim here was to identify clinical variables that are easily assessed in clinical practice which are associated with a major response to rituximab (moderate to good EULAR response, according to DAS28 values) in patients with active rheumatoid arthritis and inadequate response to anti-TNF agents or traditional DMARDs.

Methods: Rituximab (2x1g, two weeks apart) was administered to 108 patients in four different Spanish hospitals.
