Background: The Nippy Clearway is a new mechanical insufflation-exsufflation device used to assist cough.

Methods: We compared the peak expiratory flow (PEF) with the Nippy Clearway versus the CoughAssist in a bench experiment. The relationship between PEF and pressure at the airway opening during exsufflation (minimum expiratory PAO) was investigated under 6 combinations of compliance (30 or 60 mL/cm H2O) (C30 and C60) and resistance (0, 5, or 20 cm H2O/L/s) (R0, R5, and R20) over a 25-50 cm H2O range of set P(AO).

Article Synopsis
  • The study examined how expiratory flow-volume (EFV) loops relate to outcomes in COPD patients receiving mechanical ventilation in the ICU.
  • Patients were divided into quartiles based on the angle of the EFV slope during expiration, with significant differences noted in ICU and hospital stay lengths, as well as the duration of ventilation across these quartiles.
  • Although hospital and ICU mortality rates did not differ among groups, there was a negative correlation between the EFV angle and respiratory resistance/compliance, indicating that steeper angles might suggest better lung function.

The patient information and obtaining their consent before any therapeutic constitute a basis for medicine exercise. However, there are situations where the physician may be in default. In that case, concepts of benefit/risk balance, of clear and appropriate information, and of legal liability for medico-surgical staff takes all their magnitude.
