Retropharyngeal abscesses in the pediatric population can cause severe respiratory distress. We report a rare case of significant airway obstruction in a 14-month-old patient requiring rapid, emergent tracheotomy after attempts at endotracheal intubation by an experienced airway surgeon were unsuccessful. The patient was diagnosed with streptococcal pharyngitis 9 days prior to presentation to our facility and was being treated with amoxicillin.

Although generally safe and effective, severe perioperative complications, including cardiac arrest, may occur during general anesthesia in infants. With the emergence of evidence that specific anesthetic agents may affect future neurocognitive outcomes, there has been an increased focus on alternatives to general anesthesia, including spinal anesthesia. We present a case of cardiac arrest during general anesthesia in an infant who required urologic surgery.

Article Synopsis
  • The study evaluated the effectiveness of using computerized order sets and an Acute Pain Treatment Service (APS) in improving monitoring of pediatric patients receiving patient-controlled analgesia (PCA) for pain management.
  • Three patient groups were analyzed based on their PCA order sets: those supported by APS, those using a general order set, and those with no order set, totaling 536 patients.
  • Results showed that using the APS order set led to significantly better documentation of vital signs and improved recognition of critical issues like low respiration rates, highlighting the benefits of structured monitoring in pediatric pain management.
