Data on the study of activity of cysteine proteinases and their inhibitors in larynx neoplastic tissues and control mucosa surrounding them are submitted. It is established that cysteine activity was significantly increased and the content of cysteine inhibitors was decreased in the larynx cancer tissue compared to papilloma tissue and control mucosa. The ratios between cysteine activity and level inhibitors is the informative index of the trual potential proteolytic activity in cancer cells and their invasive capacity.

Information about basic mechanisms of programmed cell death (apoptosis) development with participation of proteolytic enzymes is given in the review. The basic mechanisms of apoptosis launching are conditionally subdivided into three groups, depending on the "points of application" of apoptosis development initiating factor: membrane (receptor-dependent), mitochondrial and nuclear. Attention is accentuated on ubiquitin-dependent proteasomal proteolysis as the key system regulating apoptosis.

Employment of the authors' highly sensitive biochemical method allowed the fact of resorption of proteolytic enzymes after their peroral ingestion to be confirmed, with a question having been raised as to further fate of proteinases and mechanisms of curative action of combined enzyme preparations. The analysis of relevant data from published literature and the results of the author's own studies permitted identifying the most important moments of realization of the systemic enzyme therapy effects, which are as follows: 1) activation of receptors on the surface of enterocytes; 2) formation of activated alpha 2-macroglobulin (alpha 2-M) under the action of proteinases from the intestines, and realization of catalytic activity of those enzymes involved in this restrictor of proteinases; 3) modulation of cell activity by complexes alpha 2-M-proteinase; 4) activation of the system of mononuclear phagocytes; 5) influence on the balance of bodily endogenous proteolytic systems.

Data on study of action plasma inhibitors on activity of pancreatic proteolytic enzymes (trypsin, chymotrypsin) and plant proteinases (papain, bromelain), included in composition of enzyme mixes, used for orally application are submitted. It is established, that serine proteases are more sensitive to inactivation of plasma inhibitors, than cysteine enzymes. Main inhibitor of the papain and bromelain is alpha-2-macroglobulin in complex with which they preserve significant part of initial activity.

Using clinical and biochemical data, the efficacy of the topical application of kontrikal, a polyfunctional inhibitor of proteinases, for the therapy of chronic purulent sinusitis is discussed. The drug was found to reduce the activity of proteolytic enzymes in the purulent exudate from the maxillary sinus. As compared with the controls (27 patients), the treated group (38 patients) showed a rapid disappearance of clinical manifestations, shortening of the therapeutic period, increase of proteinase inhibitors and decrease of protein in the exudate.

Simple procedure was developed for production of fibrinogen concentrate from small amounts of human autogenous blood using precipitation with polyethylene glycol and ammonium sulfate. Maximal yield of fibrinogen was obtained using polyethylene glycol 2,000 and 6,000 daltons at concentrations 7% and 4.5%, respectively.

Acute experimental intoxication in rats produced essential changes of the absorptive capacity of neutrophilic blood granulocytes, proteolytic activity, level of proteinase inhibitors and content of blood plasma fibrinogen. Female rats showed a distinct reduction of the activity of natural killer cells. Alcohol had no essential effect on the production of hemolytic antibodies as well as reagins in the respiratory organs.

Specific proteolytic and antitryptic activities were measured in the sinus exudate of 27 patients with chronic suppurative maxillary sinusitis. Most patients showed a shift in the proteinase-inhibitor system with proteolytic activity being predominant. In the course of treatment antitryptic activity in the exudate increased.

Changes in the total blood proteolytic activity, prekallikrein level, serum kallikrein inhibitors and other proteinases were studied during the growth and metastatic spreading of different experimental tumours. It has been established that the dissemination and metastatic spreading of Guérin carcinoma inoculated intratesticularly to rats is accompanied by a decrease in the content of prekallikrein and serum kallikrein inhibitors, which shows the activation of blood kinins. Changes in the total proteolysis and in prekallikrein content in mice with carcinoma 3LL and melanoma B-16 are of two-phase character: at the first stage after tumour inoculation there occurs an increase in the proteolytic activity and in prekallikrein content.
