Background: Patients are referred to functional coronary artery disease (CAD) testing based on their pre-test probability (PTP) to search for myocardial ischemia. The recommended prediction tools incorporate three variables (symptoms, age, sex) and are easy to use, but have a limited diagnostic accuracy. Hence, a substantial proportion of non-invasive functional tests reveal no myocardial ischemia, leading to unnecessary radiation exposure and costs.

Objectives: Predicting the presence or absence of coronary artery disease (CAD) is clinically important. Pretest probability (PTP) and CAD consortium clinical (CAD2) model and risk scores used in the guidelines are not sufficiently accurate as the only guidance for applying invasive testing or discharging a patient. Artificial intelligence without the need of additional non-invasive testing is not yet used in this context, as previous results of the model are promising, but available in high-risk population only.

We report the first-principles study of small polarons in the most stable two-dimensional pnictogen allotropes: blue and black phosphorene and arsenene. While both cations and anions of small hydrogen-passivated clusters show charge localization and local lattice distortions, only the hole polaron in the blue allotrope is stable in the infinite size cluster limit. The adiabatic polaron relaxation energy is found to be 0.

The article describes a clinical case of bilateral occlusion of retinal vessels in a patient with Waldenstrom's disease - a rare lymphoplasmocytic tumor of the bone marrow characterized by a complex of syndromes, among which the syndrome of blood hyperviscosity dominates. Comprehensive clinical, instrumental and laboratory examinations revealed that besides the syndrome of blood hyperviscosity the patient also had loci of cerebral ischemia (according to magnetic resonance imaging), ocular hypoperfusion with severe deficiency of retinal and choroidal blood flow (according to Doppler methods) indicating the presence of ocular ischemic syndrome. Since bilateral occlusion of retinal vessels without concomitant vascular and/or systemic pathology is rare, patients with such diagnosis should be referred to a hematologist.

The analysis of a clinical picture barometeosensitivity in a seaside zone of the Far North is carried out in the article. A diagnostic test included the following laboratory assessments: complete blood count, complete urinary analysis, biomedical measurement assessment, immunology blood research and functional renal test; analysis of the level of an electrolyte in the blood and hormone receptor status; and the following instrumental diagnostics: The auscultatory Korotkov's method of determining blood pressure, electrocardiography, variational pulsometry, chest X-ray, mechanocardiography, echocardiography and abdominal ultrasound; and also consultation of ophthalmologist and neuropathologist. Every patient should keep a diary "weather - health" before and after the treatment.

Surgical management of pathological deformities of the internal carotid arteries, a cause of chronic brain ischemia, is discussed. This pathology is very common and is found in 25% of all individuals who underwent preventive medical examination according to the ultrasonography data. Most deformities do not pose any threat to patients, while some of them may cause ischemic stroke and chronic brain ischemia.

For the optimization of luminescence dating and dosimetry techniques on the basis of the optically stimulated luminescence, the stimulation spectra of quartz and alkali feldspars were measured in the spectral region of 250-1100 nm using optically stimulated afterglow. Optically stimulated luminescence in all studied spectral regions is induced by the same kind of deep traps, that produce thermoluminescence in the regions of palaeodosimetric peaks for both minerals. The mechanism for photoionization of deep traps was proposed as being due to delocalization of the excited state of the corresponding lattice defects.

The results of treatment of 57 patients with an acute ileus (AI), caused by colonic cancer, were analyzed. The compensated (in 9 patients), sub-compensated (in 35) and decompensated (in 13) forms of AI were assigned. In the right-sided localization of tumor or left-sided one and compensated AI the one-stage radical operation was done.

The experience with surgical treatment of 89 patients with recurrent inguinal hernia is presented. A method for hernioplasty with the use of the pyramidal muscle transferred for covering the inguinal space is described. In 37 patients, no hernia recurrence and injury to the femoral vessels were revealed.

Changes of intra-abdominal pressure, the degree of abdominal wall stretching, the strength of connection of the tissues and the quality of the operative wound, central hemodynamics, and the indices of external breathing were studied depending on the type of hernioplasty in 44 animals (dogs) and 420 patients with large ventral hernias. The authors suggest a method for surgical treatment of ventral hernias by twisted aponeurotic sutures applied under control of intraabdominal pressure. With the use of this method the number of complications in the main group reduced 3.

Antibacterial properties of the Soviet drug nitazol which is a derivative of imidazole were studied. It was shown that nitazol in a dose of 4-8 micrograms/ml was highly active against gram-negative nonsporulating anaerobes, gram-positive anaerobic cocci and spore-forming Clostridia spp. Unlike metronidazole, it was efficient against both standard and clinical strains of facultative anaerobes such as E.

Therapeutic effect of liposomal dosages of rifampicin and prodigiozan was studied on rabbits with simulated chronic tonsillitis in comparison to that of commercial ones of the drugs. The treatment schemes included daily intra-tonsillar++ injections of the dosage forms for 5 days. A high efficacy of their liposomal dosage forms in treatment of experimental chronic tonsillitis was confirmed microbiologically and immunologically.

The experimental-clinical investigations have demonstrated the high effectiveness of the native preparation Nitazole against the Gram-negative non-spore-forming anaerobic bacteria, Gram-positive anaerobic cocci, spore-forming Clostridium, certain facultative anaerobes. This permits to recommend it for the use as an antibacterial preparation for the treatment of peritonitis in children.

The results of treatment of 76 patients with various diseases of the lungs complicated by hemorrhage are analysed. A complex of diagnostic and therapeutic measures were applied in a certain order, including bronchoscopy with temporary occlusion of the bronchi, bronchial arteriography with acute embolization of the bronchial artery. Hemorrhage was arrested in 73 patients with the use of such tactics, in 48 of them an economical operative intervention was carried out.

Studies of immunogenic properties of P. aeruginosa anatoxin incorporated into liposomes of varying lipid compositions have shown that single immunization of mice with the anatoxin included into phosphatidyl ethanolamine and lecithin cholesterol liposomes induces a 6-8-fold higher antitoxin antibodies production than immunization with unadsorbed or adsorbed on aluminum hydroxide P. aeruginosa toxoid.
