The increase in wildfires and bushfires due to climate change means that more people, including pregnant women and their fetuses will be exposed to landscape fire smoke. Although there is evidence to suggest that pregnancy landscape fire exposure is associated with lower birth weight, preterm birth and pregnancy loss, there is a lack of information on many other perinatal outcomes, as well as information on subsequent respiratory outcomes in children. Furthermore, due to the generally short term (hours/days) and intermittent nature of landscape fire smoke exposure, the knowledge to date has largely relied on natural experiments and ecological studies which can be subject to misclassification of exposure and a lack of precision.

Introduction: Multiple cohort studies have been established to investigate the impact of early life factors on development and health outcomes. In Australia the majority of these studies were established more than 20 years ago and, although longitudinal in nature, are inherently susceptible to socioeconomic, environmental and cultural influences which change over time. Additionally, rapid leaps in technology have increased our understanding of the complex role of gene-environment interactions in life course health, highlighting the need for new cohort studies with repeated biological sampling and in-depth phenotype data across the first 1000 days of life from conception.

Asthma is the most prevalent chronic disease in pregnancy, and as symptoms can change with pregnancy, and exacerbations of asthma are common, regular review of asthma symptoms, the provision of self-management education, and appropriate medication use are the hallmarks of management. Co-morbidities are also prevalent among pregnant women with asthma, with obesity, gestational diabetes, hypertension, rhinitis, depression, and anxiety, more common among women with asthma than women without asthma. Less is known about reflux, obstructive sleep apnoea and vocal cord dysfunction, along with nutritional deficiencies to iron and vitamin D.
