We investigated the potential role of pivmecillinam in the treatment of urinary tract infections in children. Among 351 children (0-6 years) with urinary tract infections, 83% could be treated with pivmecillinam following urine culture. Resistance was highest in infants (0-3 months) caused by the high prevalence of Enterococcus faecalis.

Article Synopsis
  • There is a significant need to reduce infections, especially urinary tract infections (UTIs), and improve antibiotic use in nursing homes amid rising antimicrobial resistance.
  • The study aims to tackle health care-associated infections through a comprehensive intervention for health care professionals in nursing homes across eight European countries.
  • A before-and-after audit approach will be used to evaluate the effectiveness of strategies, including training and feedback on infection control practices and appropriate antibiotic prescribing.

Background: Overdiagnosis of urinary tract infections (UTIs) is one of the most common reasons for the unnecessary use of antibiotics in nursing homes, increasing the risk of missing serious conditions. Various decision tools and algorithms aim to aid in UTI diagnosis and the initiation of antibiotic therapy for residents. However, due to the lack of a clear reference standard, these tools vary widely and can be complex, with some requiring urine testing.

Haemophilus influenzae is an uncommon uropathogen with fastidious growth requirements, which must be taken into consideration in the diagnostic process. We present a rare case of urosepsis with H. influenzae in a young patient with nefrocalcinosis.

Article Synopsis
  • Candidemia is a serious fungal infection linked to high mortality rates, emphasizing the need for prompt diagnosis through blood cultures, especially since fungi are often overlooked in standard sepsis treatments.
  • A study compared blood culture results from 2018 and 2020 in Denmark, noting a significant improvement in identifying fungi when a mycosis flask was added to blood culture sets, especially in high-risk patient areas.
  • The research found that including a mycosis flask increased the likelihood of detecting candidemia, with a notable difference in high-risk departments, suggesting a need for tailored blood culture strategies in those settings.

Background: Neurological complications during and after SARS-CoV-2 infection have been frequently described. The detection of either SARS-CoV-2 RNA or specific antibodies against SARS-CoV-2 in cerebrospinal fluid in the context of concomitant neurological manifestations indicates neuroinfection.

Methods And Results: This is a retrospective descriptive analysis of cerebrospinal fluids and serum samples from 2 hospitalized patients and autopsy findings from 2 patients who died at home.

Background: Mecillinam (amdinocillin) is active against Gram-negative bacteria. Clinical data on the efficacy of IV mecillinam for severe urinary tract infections is sparse.

Objectives: To assess the effectiveness of targeted IV mecillinam compared with other β-lactams for bacteraemia with Escherichia coli and Klebsiella spp.

Background: Fluoroquinolones are extensively used as prophylaxis for transrectal ultrasound-guided biopsy of the prostate (TRUBP). Emerging fluoroquinolone resistance and selection of multiresistant organisms warrant new prophylactic strategies. Pivmecillinam and amoxicillin/clavulanic acid have mutual synergistic activity and the combination of these agents has a broad coverage of the majority of microorganisms causing infectious complications after TRUBP and may be a valuable future prophylactic regimen.
