The features of the blood flow in the vertebral artery were studied by ultrasonic dopplerography with functional test for reactive hyperemia in 52 patients. The authors claim this to be an objective screening method allowing the sequelae of occlusion of the vertebrosubclavian segment to be studied and providing information for establishing the diagnosis and appraising the efficacy of reconstructive operations.

The results of 110 operations for atypical shunting in occlusions of the aorta and the iliac and femoral arteries are analysed. The authors suggest modifications of operations for transverse femoro-femoral shunting with the use of venous autografts. Shunting of lower limb arteries from the branches of the arch of the aorta made it possible to preserve the limb in 58.

The authors analyse the results of endoscopy in 127 patients and clinical observations over 98 patients with giant ulcers of the stomach. The study provided evidence of the low frequency of giant ulcers, their marked polymorphism, and tendency to bleed and degenerate. These ulcers do not differ from the usual ulcers in the values of the secretory activity of the stomach and the duodenogastric reflux, but possess characteristic morphologic criteria.

An analysis of results of the examination and surgical treatment of 310 patients have shown that in patients with high predisposition to dumping-syndrome the following operations should be used: Billroth-I resection of the stomach (for gastric ulcers), selective proximal vagotomy or vagotomy with draining operations (for duodenal ulcers). Indications for the above operations should be determined with special reference to the state of the secretory and motor-evacuatory function of the stomach.

The examination of 38 patients with recurrent and peptic ulcers after vagotomy (reconstructive operations were fulfilled in 25 out of them) has established that the main cause of recurrent and peptic ulcers developing after vagotomy is the preserved elevated secretory activity after operation and high content of gastrin in the blood. Surgery is thought to be the main method of treatment of such patients.
