[Treatment of patients with perforated pyloroduodenal ulcers].

Results of treatment of 206 patients with perforated gastroduodenal ulcers were analyzed. Atypical clinical picture of perforated ulcer in some patients led to diagnostic mistakes. Diagnostic algorithm including videolaparoscopy in difficult cases is proposed.

[Treatment of gastroduodenal ulcer bleedings in a general hospital of emergency care].

Treatment of patients with gastroduodenal ulcer bleedings in a general hospital of emergency care is a complex of measures including of endoscopic diagnosis and stopping of bleeding, use of effective antisecretory drugs with obligatory laboratory control of acid-producing function of the stomach, surgical treatment and eradication of H. pylori. Up-to-date methods of endoscopic hemostasis - spirituous infiltration, argon-plasma coagulation, clipping of vessels in ulcer and also their combination in difficult cases - provide reliable hemostasis and allow to avoid urgent surgery.

[Choice of surgery scope in occlusive obstruction of the colon].

From 1980 five hundred and seventy-three patients underwent surgery for occlusive obstruction of the colon (OOC). Radical surgeries (left-sided hemicolonectomy, Hartman's surgery, subtotal colonectomy) were performed in 440 (77%) patients, 133 (23%) patients underwent palliative surgeries. One hundred and sixty-one patients of radically operated underwent one-stage surgeries (93 right-sided hemicolonectomies and 68 subtotal colonectomies).

[Principles of treating stomach and duodenal ulcers complicated by hemorrhage].

Ulcerative gastroduodenal bleedings have remained one of the most complicated problems of emergency abdominal surgery up to the present. According to the literature, they make up approximately one half of all gastroduodenal bleedings and are accompanied by high lethality--from 10 to 30%. At that the highest percentage of lethality falls at elderly and old patients.

[Lipid peroxidation and antioxidant system functions in patients with gastroduodenal ulcerous hemorrhages undergoing conservative and surgical treatment].

The results of examination of 53 patients have shown that greater intensity of lipid peroxidation and less activity of the antioxidant system of blood are more pronounced in the operated patients. The authors recommend to include antioxidants in the complex of medical measures.

[Effect of ranitidine on the function of the gastric glucocorticoid receptors].

The authors studied ranitidine effects on the function of glucocorticoid receptors (GR) of gastric cytosol in patients with massive bleeding from gastric ulcer. Fast cytosol was obtained from the tissues taken from the great culvature of the resected stomach. GR type II and III were identified using 3H-acetonide triamcinolone and 3H-hydrocortisone with high specific activity, respectively.

[Endoscopic glue infiltration and diathermocoagulation in the complex treatment of patients with acute gastroduodenal hemorrhages].

A special medicinal glue MIK-T capable of infiltrating biological tissue by means of an endoscopic needle was developed. The authors suggest a method for glue endoscopic infiltration of the source of gastroduodenal bleeding for its arrest and prevention of a recurrence. Experimental study was undertaken to study the efficacy of glue endoscopic infiltration, determine the terms of glue resolution, and reveal the morphological changes in the zone of glue injection.

[Subtotal colectomy in the treatment of intestinal obstruction in cancer of the left half of the colon].

Eleven cases of acute obstructing carcinoma of the left colon were treated by emergency subtotal colectomy with immediate ileocolorectal anastomosis. The procedure of on-table closed antegrade preparation of the small bowel has been developed by the authors preventing the anastomosis incompetence. In all cases during postoperative treatment the long-tube decompression, enteral tube feeding and HBO were used.

[Effectiveness of hyperbaric oxygenation after operations for acute intestinal obstruction].

Positive effects of HBO sessions on indices of central hemodynamics is shown in 34 patients with different forms of acute intestinal obstruction.

[Effect of hyperbaric oxygenation on patient hemodynamics and respiration in blood loss as a consequence of gastroduodenal hemorrhages].

Hyperbaric oxygenation (HBO) was used in the complex of treatment of 52 patients who had blood loss due to gastroduodenal hemorrhage. Barocameras were used with the maximum pressure of 1.6--2.

[Results of treatment of perforated peptic ulcer by the method of primary gastric resection].

Under observation were 315 patients subjected to primary gastric resection for perforated ulcer. Twelve patients died. Late results were followed up in 171 patients.
