Article Synopsis
  • Hematoma expansion (HE) occurs in a significant portion of patients with acute intracerebral hemorrhage (ICH), impacting their outcomes; the study focuses on the predictive accuracy of the Black-&-White (B&W) sign in identifying HE.
  • In a multicenter cohort from the PREDICT study, the association between the B&W sign and HE was analyzed, revealing that patients with the B&W sign had a higher frequency of HE and more substantial growth of hematomas compared to those without it.
  • The B&W sign strongly predicts HE, with an adjusted odds ratio of 7.83 for HE and 5.67 for severe HE, indicating that its presence significantly increases the risk of hematoma expansion.

Objective: Microvascular decompression (MVD) in trigeminal neuralgia (TN) is currently managed using an operating microscope (OM). Recent experiences with endoscopy-assisted, fully endoscopic, and exoscopic surgery have been described, aiming to improve ergonomics and image quality. The aim of this study was to report the first series of patients operated on with the aid of a robotic binocular exoscope using a head-mounted display (RoboticScope).

Article Synopsis
  • * Patients are categorized into anemic and nonanemic groups, as well as hyperglycemic and normoglycemic groups, to see how these factors affect the underestimation of ischemic core volume (ICuV).
  • * Results showed that hyperglycemic patients had a higher rate of "perfusion scotoma" (65%) and a larger ICuV compared to normoglycemic patients, suggesting metabolic factors can significantly influence stroke outcomes.

Introduction: Data collection often relies on time-consuming manual inputs, with a vast amount of information embedded in unstructured texts such as patients' medical records and clinical notes. Our study aims to develop a pipeline that combines active learning (AL) and NLP techniques to enhance data extraction in an acute ischemic stroke cohort.

Materials And Methods: Consecutive acute ischemic stroke patients who received reperfusion therapies at IRCCS Humanitas Research Hospital were included.

Background Aims: Mounting evidence suggests that persistent cell expansion is the main driver for both efficacy and toxicity of chimeric antigen receptor (CAR) T-cell therapy. Hereby, we describe a case of delayed recurrent neurotoxicity associated with late CAR T-cells re-expansion.

Case Description: A 44-year-old man suffering from mantle cell lymphoma received brexu-cel.

Background: Existing radiological markers of hematoma expansion (HE) show modest predictive accuracy. We aim to investigate a novel radiological marker that co-localizes findings from non-contrast CT (NCCT) and CT angiography (CTA) to predict HE.

Methods: Consecutive acute intracerebral hemorrhage patients admitted at Foothills Medical Centre in Calgary, Canada, were included.

Background: Early ischemic change and collateral extent are colinear with ischemic core volume (ICV). We investigated the relationship between a combined score using the Alberta Stroke Program Early Computed Tomography Score and multiphase computed tomography angiography (mCTA) collateral extent, named mCTA-ACE score, on functional outcomes in endovascular therapy-treated patients.

Methods: We performed a post hoc analysis of a subset of endovascular therapy-treated patients from the Alteplase Compared to Tenecteplase trial which was conducted between December 2019 and January 2022 at 22 centers across Canada.

Soccer is one of the most popular sports worldwide, played by over 270 million people and followed by many more. Several brain health benefits are promoted by practising soccer and physical exercise at large, which helps contrast the cognitive decline associated with ageing by enhancing neurogenesis processes. However, sport-related concussions have been increasingly recognised as a pressing public health concern, not only due to their acute impact but also, more importantly, due to mounting evidence indicating an elevated risk for the development of neurological sequelae following recurrent head traumas, especially chronic traumatic encephalopathy (CTE).

Introduction: Seizures may occur in up to 30% of non-Hodgkin lymphoma patients who received anti-CD19 CAR T-cell therapy, yet the optimal anti-seizure medication (ASM) prevention strategy has not been thoroughly investigated.

Methods: Consecutive patients affected by refractory non-Hodgkin lymphoma who received anti-CD19 CAR T-cells were included. Patients were selected and assessed using similar internal protocols.

Article Synopsis
  • High-grade gliomas (HGGs) are a type of brain tumor that doctors find hard to treat, and people with these tumors usually don’t live long after being diagnosed.
  • Recently, immunotherapy, a new kind of treatment that helps the body's immune system fight cancer, has shown promise for treating many cancers, including HGGs.
  • Researchers looked at 52 studies to understand the effectiveness of different types of immunotherapy for HGGs, but found that most trials, especially the bigger ones, did not help patients live longer, which means more work is needed to improve these treatments.

Objective: The underpinning biologics of migraine chronification are not well understood. We aim to investigate the role of the cumulative burden of stress, namely the allostatic load, in migraine chronification.

Methods: This was a cross-sectional study.

Introduction: Migraine's astonishing prevalence and preserved genetic background contrast with the definition of a disease and the biological meaning of experiencing recurrent, severe headache attacks is still puzzling.

Methods: To provide a comprehensive explanation of the migraine evolutionary meaning, we review (i) the putative role of the autonomic nervous system in migraine attacks, (ii) the inter-ictal autonomic, functional, and metabolic signature of migraine patients, (iii) the bio-behavioral perspective of pain, and (iv) the allostatic perception of migraine chronification.

Results: Migraineurs have inter-ictal cortical hyperexcitability and metabolic dysfunction that predisposes to brain energetic imbalance.

Background: Carotid web (CaW) and carotid free-floating thrombus (CFFT) are rare yet critical causes of ischemic stroke in young adults.

Case Presentation: A 54-year-old woman presented with a fluctuating right sensory-motor faciobrachial syndrome. A brain MRI scan revealed multiple small recent asynchronous cortico-subcortical ischemic foci in the vascular territory of the left internal carotid artery.

Neuropathic pain, traditionally considered a chronic condition, is increasingly encountered in the emergency department (ED), accounting for approximately 20% of patients presenting with pain. Understanding the physiology and key clinical presentations of neuropathic pain is crucial for ED physicians to provide optimal treatment. While diagnosing neuropathic pain can be challenging, emphasis should be placed on obtaining a comprehensive medical history and conducting a thorough clinical examination.

Background: Migraine is a disabling and chronic neurovascular headache disorder. Trigeminal vascular activation and release of calcitonin gene-related peptide (CGRP) play a pivotal role in the pathogenesis of migraine. This knowledge has led to the development of CGRP(-receptor) therapies.

We explored temporal variations in disease burden of ambient particulate matter 2.5 μm or less in diameter (PM) and ozone in Italy using estimates from the Global Burden of Disease Study 2019. We compared temporal changes and percent variations (95% Uncertainty Intervals [95% UI]) in rates of disability adjusted life years (DALYs), years of life lost, years lived with disability and mortality from 1990 to 2019, and variations in pollutant-attributable burden with those in the overall burden of each PM- and ozone-related disease.
