Some patients develop ischemic stroke despite taking direct oral anticoagulants because of the presence of other risk factors such as coagulopathies. A 65-year-old male patient with non-valvular atrial fibrillation (NVAF) taking rivaroxaban was diagnosed as having embolic stroke and antithrombin-III (AT-III) deficiency. Echocardiography revealed a thrombus in the left atrial appendage (LAA).

Article Synopsis
  • The study investigates the impact of pre-existing and new onset left bundle branch block (LBBB) on clinical outcomes in patients undergoing transcatheter aortic valve replacement (TAVR).
  • Out of nearly 6,000 patients, 4.6% had pre-existing LBBB, while 27.6% developed new onset LBBB post-procedure.
  • Results show that pre-existing LBBB was linked to significantly higher rates of all-cause and cardiovascular mortality compared to both the new onset LBBB and no LBBB groups, highlighting the need for careful monitoring of these patients.

Dietary therapy is crucial for diabetes care with the aim of preventing the onset and progression of diabetes and its complications. The traditional approach to dietary therapy for diabetes has primarily focused on restricting the intake of the three major nutrients and rigorously controlling blood glucose levels. However, advancements in nutritional science have shown that within the three major nutrients - carbohydrates, proteins and lipids - there exist multiple types, each with distinct impacts on type 2 diabetes and its complications, sometimes even showing conflicting effects.

For the advancement of DKD treatment, identifying unrecognized residual risk factors is essential. We explored the impact of obesity diversity derived from different carbohydrate qualities, with an emphasis on the increasing trend of excessive fructose consumption and its effect on DKD progression. In this study, we utilized db/db mice to establish a novel diabetic model characterized by fructose overconsumption, aiming to uncover the underlying mechanisms of renal damage.

Accumulating evidence suggests health benefits of ketone bodies, and especially for longevity. However, the precise role of endogenous ketogenesis in mammalian life span, and the safety and efficacy of the long-term exogenous supplementation of ketone bodies remain unclear. In the present study, we show that a deficiency in endogenous ketogenesis, induced by whole-body Hmgcs2 deletion, shortens life span in mice, and that this is prevented by daily ketone body supplementation using a diet containing 1,3-butanediol, a precursor of β-hydroxybutyrate.

Article Synopsis
  • This study examined how significant tricuspid regurgitation (TR) affects patients with severe aortic stenosis who undergo transcatheter aortic valve implantation (TAVI).
  • Moderate or greater TR was found in 8% of patients, significantly increasing the risk of all-cause and cardiovascular death, especially in those with right ventricle dysfunction (RVD).
  • Patients with ventricular functional TR had the lowest survival rates, and persistent RVD post-TAVI indicated a higher risk of cardiovascular death compared to those whose RV function improved.

Background: Several trials demonstrated that aspirin monotherapy compared with aspirin plus clopidogrel is associated with a lower incidence of bleeding without an increased risk of ischemic events in patients after transcatheter aortic valve replacement (TAVR); however, there remains a paucity of data to prove the necessity of even aspirin monotherapy.

Objectives: This study aimed to compare clinical outcomes and valve performance of the 3 different antithrombotic strategies post-TAVR from the OCEAN-TAVI (Optimized transCathEter vAlvular iNtervention) registry.

Methods: Patients who received anticoagulation or had procedural complications were excluded.

Background Subclinical leaflet thrombosis, characterized by hypoattenuated leaflet thickening (HALT) on multidetector computed tomography, is common after transcatheter aortic valve replacement (TAVR). Because little is known about the long-term natural history of subclinical HALT, we aimed to investigate this in patients who underwent TAVR without using additional anticoagulation. Methods and Results We retrospectively evaluated patients who underwent TAVR with the Edwards SAPIEN-XT at our institute between October 2013 and December 2015.
