Background: Epidural blood patch (EBP) is frequently used for the treatment of spontaneous intracranial hypotension (SIH) and anesthesiologists are often involved in performing such procedures. However, the optimal technique and approach of EBP remains uncertain.

Methods: This case series included adult patients with SIH who underwent EBPs at London Health Science Centre, Ontario, Canada between 2010 and 2022.

Myoclonus has been described rarely as an adverse effect with some non-steroidal anti-inflammatory drugs, but never with indomethacin. Indomethacin is a common nonsteroidal anti-inflammatory drug used for various primary headache disorders, including hemicrania continua. We present a rare case of a 45-year-old male with hemicrania continua who developed myoclonus from indomethacin.

Cerebellar tonsillar descent can be seen on head magnetic resonance imaging in both Chiari malformation type 1 and spinal cerebrospinal fluid leak creating the potential for misdiagnosis. We report five cases of spinal cerebrospinal fluid leak at Stanford University initially misdiagnosed and treated as Chiari malformation type 1 based on cerebellar tonsillar descent demonstrated on imaging. All five cases had sustained relief at the 6-month follow up visit from epidural blood patches for the treatment of spinal cerebrospinal leak after unsuccessful suboccipital decompression surgeries.

Article Synopsis
  • Spinal cerebrospinal fluid (CSF) leaks can cause intracranial hypotension, which is important to consider for patients with sudden chronic headaches; a rare cause of this leak could be a pars interarticularis (PI) fracture.* -
  • In a study of 6 patients from 2016 to 2019, all had suspected spinal CSF leaks and diagnosed PI fractures; after undergoing targeted epidural patches with fibrin sealant, they showed significant headache improvement.* -
  • The research demonstrates the importance of recognizing PI fractures in spinal CSF leaks, the usefulness of CT scans for identifying fractures, and the effectiveness of targeted treatments at fracture sites.*
