The efficacy of functional endoscopic sinus surgery for the treatment of chronic sinusitis is well established. Two contrasting European techniques were introduced to North America in the 1980s: that espoused by Wigand, which included endoscopic isthmus modification of all sinus ostia and cavities with modification of the middle turbinates, and that developed by Messerklinger, who practiced only minimal opening of the narrow osteomeatal tract at the anterior ethmoid sinus to achieve physiologic reversal of sinus disease. These techniques were additionally modified recently by the introduction of power resection techniques by Setliff and Christmas and stereotactic computer navigation by Anon and Fried.

Computed tomography, endoscopic optical instrumentation, powered microdebriders, and computer-assisted surgery have surmounted the technical problems of obstructive sinusitis care. Most experienced practitioners achieve reported success rates of 85-95% among their surgical patients.

Generally accepted standards regarding the materials that should be used for packing, how long the packing should be left in place, or the indications for nasal packing are lacking. In view of the present lack of standardization and the many different packing materials used, we review the currently available materials and outline their respective properties, indications, and risks.
View Article and Find Full Text PDFThe ultimate success or failure of frontal sinus surgical procedures, whether they be endonasal or external, is determined essentially by the rate of restenosis of the frontal sinus outflow tract or neo-ostium postoperatively. Long-term stenting for a period of several months significantly reduces the rate of restenosis, particularly in difficult cases. We retrospectively reviewed the cases of 12 patients who received 21 frontal nasal stents, which were left in place for 6 months.

Symptoms usually are related to disruption of the normally laminar flow of air through the nasal passages. Crusting, bleeding, parosmia, and neuralgia can develop, leading the patient to seek medical care.

Our simplified protocol for the management of malar complex fractures of facial bones has shortened the operating time by one third to one fourth, produced consistent, satisfactory results, and reduced intraoperative and postoperative complications. In addition, with moderate training of residents, it has been installed as the primary method of treatment of this type of facial fracture in a residency training program that has a heavy load of these patients.
