  • - The study aims to improve the accuracy of using the inferior petrosal sinus to peripheral ACTH gradient (IPS:P) test in differentiating between Cushing's disease (CD) and ectopic ACTH syndrome (EAS) before and after desmopressin stimulation.
  • - Out of 50 patients studied, the IPS:P test showed high sensitivity rates ranging from 85.1% to 97.9% depending on the cutoff values used, with a consistent specificity of 100%.
  • - The results indicate that desmopressin stimulation enhances the diagnostic reliability of the IPS:P test, leading to quicker and more accurate responses in identifying ACTH-dependent Cushing's syndrome.

Hypopituitarism is a disorder characterized by insufficient secretion of one or more pituitary hormones. New etiologies of hypopituitarism have been recently described, including head trauma, cerebral hemorrhage, and drug-induced hypophysitis. The investigation of patients with these new disorders, in addition to advances in diagnosis and treatment of hypopituitarism, has increased the prevalence of this condition.

Treatment with rosiglitazone has been associated with severe paradoxical HDL-c reductions. To our knowledge, there are very few reports of this reaction occurring when patients are treated without the combination of a fibrate. A case of severe HDL-c lowering in a patient treated with rosiglitazone without a fibrate is presented.
