Background: Data on burden of invasive pneumococcal disease (IPD) are lacking from Southeast Asia. With the introduction of higher-valency pneumococcal conjugate vaccines (PCVs), we aim to assess the impact of PCV introduction into Singapore's National Childhood Immunisation Schedule (NCIS) on IPD burden and serotype replacement.

Methods: Using IPD surveillance data from a single center, we compared the incidence of IPD in children aged <17 years before and after introduction of the PCV into the NCIS (2000-2009, period 1; 2010-2023, period 2).

Aim: Continuous monitoring of blood culture (BC) systems allows rapid detection of microbial growth. We aimed to determine differences in time to positivity (TTP) in BACTEC BC between organisms and whether a 36-h period was sufficient to detect all relevant pathogenic bacteria for children admitted to a tertiary care paediatric hospital.

Methods: This was a retrospective audit of positive aerobic (AE) and anaerobic (AN) BC from paediatric inpatients with available TTP from 1 August 2016 to 2 January 2019.

Introduction: Recent reports have described the increasing predominance of Gram-negative organisms among invasive bacterial infections affecting preterm infants. This changing pattern of infections is concerning due to the spread of antibiotic resistance among Gram-negatives.

Method: We conducted a single-centre, retrospective cohort study involving very-low-birthweight (VLBW) (<1500 grams) infants born <32 weeks gestation, with culture-proven infections (blood, urine, cerebrospinal fluid [CSF]) in the neonatal intensive care unit from 1 January 2005 to 31 October 2017.

The COVID-19 pandemic led to an initial increase in the incidence of carbapenem-resistant Enterobacterales (CRE) from clinical cultures in South-East Asia hospitals, which was unsustained as the pandemic progressed. Conversely, there was a decrease in CRE incidence from surveillance cultures and overall combined incidence. Further studies are needed for future pandemic preparedness.

Objectives: Vancomycin 24-hour area under the curve over minimum inhibitory concentration (AUC/MIC) monitoring has been recommended over trough-based monitoring in pediatric patients. This study compared the proportion of target attainment between vancomycin AUC/MIC and trough-based methods, and identified risk factors for subtherapeutic initial extrapolated targets.

Methods: This was a retrospective, observational study conducted at KK Women's and Children's Hospital (KKH), Singapore.

Background And Aims: There is a paucity of information on remdesivir (RDV) use in severe pediatric coronavirus disease 2019 (COVID-19). We aimed to explore the effectiveness of RDV as the cumulative proportion of pediatric COVID-19 patients deescalated from Day 5 of high dependency or intensive care unit (HD/ICU).

Methods: All children ≤18 years admitted to Singapore's largest pediatric hospital from January 1, 2020 to March 18, 2022 were reviewed retrospectively.

The Omicron variant has been reported to present with milder disease compared with Delta, although this may be due to immunity from vaccination and prior exposure. Predictors of severity with recent strains have not been well characterized. We retrospectively examined consecutive cases of moderate-to-severe COVID-19 (defined as requiring supplemental oxygenation, intensive care or mortality) admitted to seven tertiary hospitals across Singapore in April 2023.

Objective: The aim of this study was to describe the time series of broad-spectrum antibiotic utilisation and incidence of antibiotic-resistant organisms during the implementation of antimicrobial stewardship programmes (ASP) in Singapore.

Methods: An observational study was conducted using data from 2011 to 2020 in seven acute-care public hospitals. We applied joinpoint regressions to investigate changes in antibiotic utilisation rate and incidence density of antibiotic-resistant organisms.

Introduction: Multisystem inflammatory syndrome in children (MIS-C) is a rare inflammatory syndrome with multisystem involvement affecting children exposed to COVID-19. This condition is rarely reported in East Asia and was not detected in Singapore until 2021. We present 12 cases of MIS-C diagnosed in KK Women's and Children's Hospital (KKH) from October 2021 to December 2021.

Article Synopsis
  • A study evaluated the effectiveness of universal antigen rapid tests (ART) in preventing COVID-19 spread among hospitalized children aged 5 and older during the Omicron wave.
  • Out of 444 hospitalized pediatric patients, ART showed a high accuracy with 93.7% agreement with PCR tests, and sensitivity of 83.3% and specificity of 97.5%.
  • Findings suggested that while ART screening could help limit COVID-19 outbreaks in hospitals, its effectiveness is enhanced when combined with clinical symptoms and exposure assessment.

Article Synopsis
  • - The study aimed to create and test a predictive model that assesses the likelihood of children with COVID-19 developing severe or critical symptoms, using data from five pediatric hospitals in Asia.
  • - Researchers analyzed registry data from 1,147 COVID-positive patients to identify factors like age, comorbidities, and symptoms that increase or decrease the risk of severe illness.
  • - The model showed strong predictive ability, with high accuracy rates in both training and validation tests, indicating it could help estimate risk in hospitalized children, but further validation is needed for clinical application.

Pertussis vaccination (Tdap -Tetanus-diphtheria-acellular pertussis) for pregnant women has been recommended since November 2017 in Singapore. In this prospective test-negative case-control study from 2018 to 2019, we aimed to evaluate vaccine effectiveness (VE) against pertussis infection and pertussis-related intensive care unit (ICU) admission according to Tdap (Tetanus-diphtheria-acellular pertussis) during pregnancy and/or infant pertussis vaccination. A total of 58 children (26 cases, 32 controls) were recruited with 4 ICU admissions.

Article Synopsis
  • Carbapenemase-producing Enterobacterales (CPE) infections in hospitals are thought to spread mainly from detected carriers to other patients in the same ward, but there is limited information on how plasmid-mediated transmission occurs at a larger scale.
  • A study conducted in Singapore over 4.7 years analyzed 779 patients with CPE, finding that 42% of cases were due to clonal transmission, while 44.8% were linked to plasmid-mediated transmission.
  • The results indicated that while direct contact in wards saw a decrease in transmission, indirect contacts (with no overlapping admission periods) remained high, highlighting the need for new strategies to tackle both clonal and plasmid-based infections in hospitals, as the latter

Introduction: Severe Combined Immunodeficiency (SCID) is generally fatal if untreated; it predisposes to severe infections, including disseminated Bacille-Calmette-Guerin (BCG) disease from BCG vaccination at birth. However, delaying BCG vaccination can be detrimental to the population in tuberculosis-endemic regions. Early diagnosis of SCID through newborn screening followed by pre-emptive treatment with anti-mycobacterial therapy may be an alternative strategy to delaying routine BCG vaccination.
