A multimodal treatment strategy for recurrent breast cancer is described. Occasionally, there are a few patients who have no other detectable distant metastases at the time of locoregional recurrence or of appearance of solitary lung metastasis. In such cases, curative results may be obtained with surgical treatment.

Changes in the distribution of the hepatic blood flow induced by intra-arterial infusion of angiotensin II (AT-II) were studied in human hepatic cancers using extremely short-lived radioisotope (RI) (krypton 81 m [81mKr]; half-life, 13 seconds). After the start of continuous infusion of AT-II, the radioactivity of the tumor showed about a two-fold increase, whereas that of the nontumor region decreased to about one half as much as the level before the infusion. Consequently, the mean ratio of the arterial blood flow in the tumor region to that in the nontumor region (T/N ratio) increased to 3.

A new metabolite was produced from 18-hydroxydeoxycorticosterone by the cytochrome P-450(11) beta linked hydroxylase system purified from bovine adrenocortical mitochondria. It was identified as 18,19-dihydroxydeoxycorticosterone by chemical synthesis on the basis of high-performance liquid chromatography, gas chromatography-mass spectrometry, and proton nuclear magnetic resonance (1H NMR) spectroscopy, and detailed structural analysis of it was performed by 1H NMR spectroscopy. The methylene protons at the C-19 position of the steroid were nonequivalent and coupled with each other, having a coupling constant of 10.

Using methylcellulose clonal cell cultures, we examined human erythroid burst-promoting activity (BPA) in sera obtained from umbilical cord blood. BPA was measured on the basis of the number of erythroid bursts (BFU-E) and hemoglobin synthesis when peripheral blood mononuclear cells were cultured with cord blood serum in the presence of 1.0 U/ml erythropoietin (Epo) at low fetal calf serum concentration (10%).

The clinical efficacy of two combination chemotherapies, "CAF" (cyclophosphamide, adriamycin, fluorouracil) and "CMcF" (cyclophosphamide, mitomycin C, fluorouracil) were evaluated on the patients with disseminated breast cancer. Two dosage levels were prepared for each combination. After the investigators decided the dosage levels for the patients, patients were assigned to either combination chemotherapy, CAF or CMcF.

Affinity of 18,19-dihydroxydeoxycorticosterone (18,19-diOH-DOC) and 18-hydroxy-19-nor-deoxycorticosterone (18-OH-19-nor-DOC) to aldosterone receptor and their mineralocorticoid activity were evaluated. 18,19-DiOH-DOC (1 X 10(-6) M) did not show appreciable binding to the receptor and its relative potency as a mineralocorticoid was estimated to be less than 1/8,000 of that of deoxycorticosterone (DOC). 18-OH-19-nor-DOC bound to the receptor with an affinity similar to that of 18-hydroxydeoxycorticosterone (18-OH-DOC) or spironolactone.

In 37 cases of locally advanced breast cancer, arterial administration of adriamycin (ADR), given a total dose of 150 mg in a single dose of 30 mg or 50 mg, was attempted as means of preoperative chemotherapy, and its tumor contraction action in the primary breast cancer and side effects were studied. On the basis of the "Assessement Criteria of Therapeutic Effects on Advanced and Recurrent Breast Cancer" by the Japan Breast Cancer Society, complete response (CR) was noted in 3 cases, and partial response (PR) in 25 cases, with an efficacy rate (CR+PR) of 75.7% (28/37 cases).

In an attempt to enhance effects of cancer chemotherapy, Angiotensin II (AT), a vasoconstricting catecholamine, was applied to intra-arterial infusion of the drug for advanced breast cancer. Quantitative measurement of tumor blood flow was conducted with scintigraphic techniques (81mKr). Angiotensin II given continuously through the internal thoracic artery induced prompt responses in local blood flow with a decrease in normal breast tissue and a concomitant increase in tumor.

In order to investigate the pathogenesis of juvenile chronic myelogenous leukaemia (J-CML) we examined the biosynthetic rates of G gamma and A gamma globin chains in the erythropoietic bursts from the bone marrow of a patient with J-CML. Combination chemotherapy with cyclophosphamide (CPA), adriamycin (ADR) and 5-fluorouracil (5-FU) (CAF therapy) was performed in 85 patients with advanced and recurrent breast cancer. According to the response criteria for advanced breast cancer by the Japan Mammary Cancer Society, complete response was obtained in 6 cases and partial response in 19 and an overall effective rate was 29.4% (25/85).

A case of leiomyosarcoma of the pancreas, occurring in a 44-year-old male, is reported. With a preoperative diagnosis of cystadenocarcinoma, the patient underwent surgical resection of a cystic tumor in the head of the pancreas. There were no metastases noted at the time.
