Hypertrophic cardiomyopathy is characterized by significant left ventricular wall thickening, often leading to obstructive symptoms. Alcohol septal ablation (ASA) has emerged as an effective treatment for patients with hypertrophic obstructive cardiomyopathy (HOCM) who remain symptomatic despite maximal medical therapy. However, the detailed long-term effects of ASA in Japanese patients with HOCM remain unclear.

Article Synopsis
  • - The study analyzed the link between the number of guideline-directed medical therapy (GDMT) drugs and mortality in heart failure patients, involving 3,146 participants divided based on their prescribed medications at discharge.
  • - Findings showed that patients with 5 or more GDMT drugs and fewer than 4 non-GDMT (ni-GDMT) drugs had the lowest mortality rates, while those with less GDMT and more ni-GDMT drugs had the highest mortality rates over a three-year period.
  • - The results emphasized that the number of ni-GDMT drugs significantly impacted three-year mortality, suggesting that managing ni-GDMT is essential for improving outcomes in heart failure treatment, while the amount of GDMT alone did not worsen

Macrophages are the primary cell type orchestrating bioresorbable vascular graft (BVG) remodeling and infiltrate from three sources: the adjacent native vessel, circulating blood, and transmural migration from outer surface of the graft. To elucidate the kinetics of macrophage infiltration into the BVG, we fabricated two different bilayer arterial BVGs consisting of a macroporous sponge layer and a microporous electrospun (ES) layer. The Outer ES graft was designed to reduce transmural cell infiltration from the outer surface and the Inner ES graft was designed to reduce cell infiltration from the circulation.

Aims: Plasma volume status (PVS), a measure of plasma volume, has been evaluated as a prognostic marker for chronic heart failure. Although the prognostic value of PVS has been reported, its significance in patients with acute decompensated heart failure (ADHF) admitted to the cardiovascular intensive care unit (CICU) remains unclear. In this study, we examined the relationship between PVS and long-term mortality in patients with ADHF admitted to the CICU.

Article Synopsis
  • Pimobendan is known to help with the symptoms of heart failure, but there's little evidence showing it improves long-term outcomes.
  • This study evaluated 175 heart failure patients, finding that those with reduced ejection fraction who received guideline-directed medical therapy (GDMT) alongside pimobendan had lower rehospitalization rates compared to those who didn't get GDMT.
  • The results suggest that adding GDMT may be beneficial for specific heart failure patients, but more extensive research is required to confirm these findings.

Red blood cell (RBC) transfusion therapy is often performed in patients with acute heart failure (AHF) and anemia; however, its impact on subsequent cardiovascular events is unclear. We examined whether RBC transfusion influences major adverse cardiovascular events (MACE) after discharge in patients with AHF and anemia. We classified patients with AHF and anemia (nadir hemoglobin level < 10 g/dL) according to whether they received RBC transfusion during hospitalization.

Article Synopsis
  • This study investigates the role of fractional excretion of urea nitrogen (FEUN) in identifying worsening renal function (WRF) and its relationship with outcomes in patients with acute heart failure (AHF).
  • Among 1,103 AHF patients, those with WRF had significantly higher rates of heart failure readmission, particularly when FEUN values were low or high.
  • The findings suggest that monitoring FEUN can help predict which AHF patients may have poor outcomes related to renal function, emphasizing its clinical importance in managing these patients.

Background: Lower extremity artery disease is strongly associated with morbidity and is typically addressed through revascularization interventions. We assessed the clinical outcomes of patients with chronic limb-threatening ischemia (CLTI) without revascularization who did and did not undergo repetitive hyperbaric oxygen therapy (HBOT).

Methods: Between April 2002 and March 2017, the records of 58 patients with CLTI (Rutherford classification 4 in 19% and 5 in 81%) were evaluated retrospectively.

Although the primary percutaneous coronary intervention (PCI) is an established treatment for acute ST-elevation myocardial infarction (STEMI), relevant guidelines do not recommend it for recent-STEMI cases with a totally occluded infarcted related artery (IRA). However, PCI is allowed in Japan for recent-STEMI cases, but little is known regarding its outcomes. We aimed to examine the details and outcomes of PCI procedures in recent-STEMI cases with a totally occluded IRA and compared the findings with those in acute-STEMI cases.

Background And Purpose: An increased number of pathogenic variants have been described in mitochondrial encephalomyopathy lactic acidosis and strokelike episodes (MELAS). Different imaging presentations have emerged in parallel with a growing recognition of clinical and outcome variability, which pose a diagnostic challenge to neurologists and radiologists and may impact an individual patient's response to therapeutic interventions. By evaluating clinical, neuroimaging, laboratory, and genetic findings, we sought to improve our understanding of the sources of potential phenotype variability in patients with MELAS.

Background: Thromboangiitis obliterans (TAO) can lead to the development of critical limb-threatening ischemia (CLTI). Despite conventional treatments, such as smoking cessation or revascularization, young patients (<50 years) still require limb amputation. Therapeutic angiogenesis using bone marrow-derived mononuclear cell (BM-MNC) implantation has been tested and shown to have reasonable efficacy in CLTI.

Standard non-invasive methods for diagnosing and selecting the best treatment for osteomyelitis in patients with multiple chronic conditions remain to be established. We aimed to evaluate the ability of quantitative Ga-citrate single-photon emission computed tomography ( Ga-SPECT/CT) to determine the indication for either non-surgical treatment or osteotomy in patients with lower-limb osteomyelitis (LLOM) associated with diabetes mellitus and lower-extremity ischemia, based on monitoring of inflammatory activity in bone tissue. This single-centre prospective study conducted from January 2012 to July 2017 included 90 consecutive patients with suspected LLOM.
